改良无张力阴道悬吊术应用于女性压力性尿失禁的临床研究

摘　　要：目的探讨改良无张力阴道悬吊术(TVT-O)应用于女性压力性尿失禁临床效果。方法选取我院2010年6月至2012年7月收治女性压力性尿失禁患者100例,采用随机数字表法分为对照组和观察组,每组各50例;其中对照组患者采用阴道无张力尿道中段悬吊术治疗;观察组患者采用经闭孔无张力尿道中段悬吊术治疗;比较两组患者临床治愈率,手术时间,术中出血量及并发症发生率等。结果对照组和观察组患者临床治愈率分别为94.0%,96.0%;两组患者临床治愈率组间比较无显著差异(P<0.05);观察组患者手术时间及术中出血量均明显少于对照组,组间比较差异显著(P<0.05);对照组和观察组患者并发症发生率分别为6.0%,8.0%;两组患者并发症发生率组间比较无显著差异(P<0.05)。结论与传统无张力阴道悬吊术相比,改良无张力阴道悬吊术(TVT-O)应用于女性压力性尿失禁临床效果确切,可有效缩短手术时间,减少创伤,且无严重并发症,具有临床使用价值。Objeetive To investigate the therapeutic effect of tension-free vaginal tape for surgical improvement used in women with stress urinary incontinence. Methods 100 patients with stress urinary incontinence were chose in the period from June 2010 to July 2012 in our hospital and randomly divided into two groups including control group （50 patients） with tension-free vaginal tape for surgical and observation group （50 patients） with improved tension-free vaginal tape for surgical; and the clinical cure rate, operative time, blood loss, and complications incidence of both groups were compared. Results The clinical cure rate of control group and observation group were separately 94.0%, 96.0%; there was no significant difference in the clinical cure rate between control group and observation group （P〈0.05） . The operative time and blood loss of observation group were significantly lower than control group （P〈0.05） .The complications incidence of control group and observation group were separately 6.0%, 8.0%; there was no significant difference in the complications incidence between control group and observation group （P〈0.05） . Conclusion Compared with tension-free vaginal tape for surgical, improved tension-free vaginal tape for surgical in women with stress urinary incontinence possess perfect effects, can efficiently reduce operative time and blood loss, has no serious complication, and is worthy of clinical application value.
