360°环形融合术(PLIF+PLF)在成人腰椎滑脱中的临床研究

摘　　要：目的探讨360°环形融合术(PLIF+PLF)治疗成人腰椎滑脱的临床疗效。方法选择2004～2012年于我院接受手术治疗的96例腰椎滑脱患者,随机分为360°环形融合术(PLIF+PLF)组和后外侧融合术(PLF)组,每组48例,回顾性分析并比较两组患者在腰椎融合率、腰椎前凸角、JOA评分改善率的差异。结果 360°环形融合术组患者的腰椎融合率、腰椎前凸角、JOA评分改善率优于后外侧融合术组,两组相比差异显著(P<0.05)。结论 360°环形融合术治疗成人腰椎滑脱能更好地保持腰椎生理前凸,获得更高的腰椎融合率,提高JOA评分,值得进一步探讨和推广。Objective To discuss the clinical effect of the 360° annular arthrodesis （PLIF＋PLF） in curing adult lumbar spondylol-isthesis. Methods The 96 patients suffering from the lumbar spondylol-isthesis who were in our hospital from 2004 to 2012 and received operation treatment were divided into the 360° annular arthrodesis （PLIF＋PLF） group and the posterolateral fusion （PLF） group at rand-om. Every group contained 48 cases. The difference of lumbar fusi rate, lumbar lordosis angle and the improvement rate of JOA score between the two groups were analyzed retrospectively and compared. Results The lumbar fusion rate, lumbar lordosis angle and the improvement rate of JOA score of 360° annular arthrodesis group were better than those of the posterolateral fusion group, and the difference was significant in statistic between the two groups （P 〈 0.05）. Conclusion The 360° annular arthrodesis in curing adult lumbar spondylolisthesis can better keep the lumbar physiological lordosis and obtain higher lumbar fusion rate and improve the JOA score.It is worth further study and promotion.
