多巴酚丁胺负荷超声心动图在儿童心肌炎诊断中的应用

摘　　要：目的　观察多巴酚丁胺负荷超声心动图对诊断儿童病毒性心肌炎的能力。方法　 31例儿童患者 ,用低剂量〔5～2 0 ︼g/ (kg· min)〕多巴酚丁胺负荷超声心动图 ,观察心肌收缩功能的变化。结果　 1 7例左室心肌收缩功能减低 ,室壁收缩期增厚率、左室短轴缩短率、射血分数、每搏量、心输出量均下降 ,即用负荷试验诊断病毒性心肌炎 1 7例 ,排除心肌炎 1 3例 ,﹀受体功能亢进 1例。结论　多巴酚丁胺负荷试验检测心肌收缩功能减低 。Objective To study the application of dobutamine stress echocardiography (DSE)in the diagnosis of children with viral myocarditis.Methods Low dose of dobutamine [5 to 20 μg/(kg·min)]stress echocardiogaphy was used to observe the left ventricular myocardiac contractive functing in 31 children with viral myocaritis.Results Seventeen children with viral myocarditis had left ventricular myocardiac contractive function reduction(△Th%,FS%,EF,SV,CO).Seventeen patients were diagnosed having viral myocarditis,13 excluded and l having β receptor hyperfunction.Conclusion DSE could identify myocardiac contractive function reduction,which might play an important role in the diagnosis of viral myocarditis.
