开环式分段黏膜切除吻合术治疗脱垂性痔病的临床应用研究

摘　　要：目的探讨开环式分段黏膜切除吻合术、PPH手术和传统Milligan—Morgan术3种术式治疗脱垂性痔病的临床疗效。方法选取2013年1~4月间在我院住院治疗的环状混合痔患者90例，随机分为治疗组（TsT组）、对照A组（PPH组）和对照B组（Milligan—Morgan术组），每组各30例。观察3组间患者围手术期的一般情况及术后并发症。结果治疗组在手术时间上明显少于对照A组及对照B组，有显著性差异（P〈0．05）；治疗组在术后第2、4、6天疼痛及水肿积分上均少于对照A组及对照B组，有显著性差异（P〈0．05）；治疗组在术后7天内出现尿潴留、急便感的情况例数均少于对照A组及对照B组。有显著性差异（P〈0．05）。结论开环式分段黏膜切除吻合术治疗脱垂性痔病比PPH手术和传统Milligan—Morgan术更具优势。Objective To study the clinical effect of tissue-selecting therapy stapler and procedure forprolapsed and hemorrhoids （PPH） and Milligan Morgan in the treatment of hemorrhoids. Methods 90 patients with circularity mixed hemor- rhoid during Jan 2013 to Apt 2013 in our hospital were randomly divided into the experimental group （TST group）,the control group A（PPH group） and control group B（Milligan Morgan group）. Observed the general situation and complications of the two groups during the intraoperative and afteroperation. Results Compared with control group A and control group B, the experi- mental group spent less time on the surgery. There was statistically significant difference （P〈0.05）. Compared with control group A and control group B, the degrees of pain and edema of the experimental group on day 2, 4, and 6 after the surgery were less. There was statistically significant difference （P〈0.05）. Compared with control group A and control group B, the cases of uroschesis and urgent urination of the experimental group within the seven days after the surgery were less. There was statistical- ly significant difference （P〈0.05）. Conclusion Compared with PPH surgery and the traditional Milligan-Morgan surgery, TST shows a great advantage in treating prolapse hemorrhoids.
