血清总胆汁酸水平在肝硬化、肝癌和梗阻性黄疸中的临床价值

摘　　要：目的探讨血清总胆汁酸水平在肝硬化、肝癌和梗阻性黄疸中的临床价值。方法选240例肝胆病患者作为病例组；选160例健康体检者作为对照组。两组TBA检测均采用酶循环法。并同时检测GGT、ALP、AST、ALT的活性指标。分析检测结果并比较。结果病例组与对照组的血清TBA水平比较，P〈0．01，前者明显高于后者；肝硬化组（阳性率100％）、梗阻性黄疸组（阳性率100％）与肝癌组（阳性率80％）的血清TBA水平比较，P〈0．01，前者明显高于后者；病例组的血清TBA水平的阳性率与传统肝功能酶学指标（GGT、ALP、AST、ALT）比较，前者高于后者，P〈0．01。结论TBA作为肝细胞损伤和胆道梗阻的检测指标，具有较高的灵敏度和特异性，优于常规的检测指标，是一种良好的检测指标。TBA水平为肝胆疾病的早期诊断提供了有利依据，给病情检测带来了重要的临床价值。同时应联合检测其他肝功能指标以提高肝胆疾病的检测率。Objective To investigate the serum total bile acid level in liver cirrhosis, liver cancer and the clinical value of obstructive jaundice. Methods Selected 240 cases of liver disease as case group; Selected 160 healthy physical examination as a control. Two groups of associates detection method of enzyme in each cycle. Detection of GGT and ALP, AST, ALT activity index. Analysis and compare the test results. Results Case group compared with control group of serum levels of associates, P 〈 0.01, the former is significantly higher than the latter; Cirrhosis of the liver （positive rate 100%）, obstructive jaundice group （positive rate 100%） and liver cancer group （positive rate 80%）, serum levels of associates P 〈 0.01, the former is significantly higher than the latter; Cases of the positive rate of serum levels of associates with traditional index of liver enzyme （GGT and ALP, AST and ALT） comparison, the former is higher than the latter, P 〈 0.01. Conclusions Associates as indicators of Liver cells injury and detection of biliary obstruction, have higher sensitivity and specificity and is superior to the conventional detection indexes, is a kind of good detection index. Associates level provides a good basis for the early diagnosis of liver disease, bring disease detection has important clinical value. At the same time, other liver function indicators should be united detection in order to improve the detection rate of liver and gallbladder disease.
