不同剂量阿托伐他汀治疗老年颈动脉内膜中层增厚或斑块形成的临床研究

摘　　要：目的探讨阿托伐他汀10mg,qd及20mg,qd治疗60～80岁且伴颈动脉内膜中层(IMT)增厚或斑块形成的老年患者的安全性及有效性。方法采用重复测量设计的方差分析,将符合纳入标准的患者共140例随机分为2组,分别给予阿托伐他汀10mg,qd或20mg,qd治疗,连续用药6个月;分别于治疗前及治疗24周后检测血脂、血清肌酐(cr)、血清谷丙转氨酶(ALT)、血清谷草转氨酶(AST)及血清肌酸激酶(CK)水平、IMT厚度或斑块大小变化及不良反应。结果治疗6个月后,2组LDL-C和TC与治疗前相比差异有统计学意义(P<0.05),双侧颈总动脉和颈动脉分叉处IMT与治疗前组内比较差异有统计学意义(P<0.05),但颈内动脉IMT与治疗前比较差异无统计学意义(P>0.05)。结论阿托伐他汀10mg,qd或20mg,qd对60～80岁且伴IMT增厚或斑块形成的老年患者的降脂疗效显著,不良反应较少,安全性较好;但对IMT的作用在颈动脉各部位的改善效果存在一定的差异。Objective To study the efficacy and safety of the atorvastatin 10 mg,qd,and 20 mg,qd on carotid intima-media thickness（IMT） and plaque formation of older patients.Methods By analysis of variance of repeated measurement design,140 cases of patients were randomly divided into two groups,and atorvastatin 10 mg,qd or 20 mg,qd were continuous administrated for 6 months.Before and 24 weeks after treatment,detection of blood lipid,serum creatinine（Cr）,serum alanine aminotransferase（ALT）,serum aspertate aminotransferase（AST） and serum creatine kinase（CK） levels,thickness of IMT and plaque size change and adverse reactions were detected.Results Six months after treatment,LDL-C and TC were significantly different with these before treatment（P0.05）,bilateral common carotid artery and carotid artery bifurcation IMT were significantly different with those before treatment（P0.05）,but the internal carotid artery IMT was not significantly different compared with that before treatment（P0.05）.Conclusion Atorvastatin 10 mg,qd or 20 mg,qd may has significant curative effect on reducing lipid in older patients with IMT thickening or plaque formation,with less adverse reaction,better security;But for the improvement of different parts of the role of IMT in carotid artery,the effect exists certain differences.
