下颌下腺内注射BTX-A时的解剖定位

摘　　要：目的探讨国人下颌下腺注射A型肉毒毒素（BTX-A）时的解剖定位。方法大体解剖观察成人下颌下腺20例标本。结果下颌下腺呈三角形或扁椭圆形,位于下颌下三角内,其上-下、前-后、内-外直径的平均值分别为27.6±3.2 mm、29.1±4.2 mm、14.5±3.6 mm。下颌角距离颏下点的平均长度为91.6±3.5 mm,下颌下腺的近端和远端距离下颌角的平均距离分别为10.6±4.3 mm和39.8±3.5 mm。面动脉在距离下颌角12.3±2.6mm处穿出下颌下腺,并且在距离下颌角26.2±3.3 mm处与下颌骨相交。结论下颌下腺内注射BTX-A的安全区域建议在腺体的外下部,距离下颌体下缘1.4~1.5 cm处的范围内,针尖进入皮肤的深度1.8~1.9 cm。Objective This study was to determine the anatomical localization of the submandibular gland for a botulinum toxin-A injection in China.Methods Twenty submandibular glands were examined.Results The shape of the submandibular gland was generally triangular or irregular round on the submandibular triangle.The mean superior-inferior diameter,anterior-posterior diameter and medial – lateral diameter of the gland were 27.6 ±3.2 mm,29.1 ±4.2 mm and 14.5 ±3.6 mm,respectively.The mean length of a line between the angle of the mandible and the gnathion was 91.6 ± 3.5 mm,the proximal and distal point of the submandibular gland from the angle of the mandible were 10.6 ± 4.3 mm and 39.8 ± 3.5 mm,respectively.The facial artery came out of the submandibular gland at 12.3 ± 2.6 mm and the position of the intersection of the facial artery with the inferior border of the mandible was located at 26.2 ± 3.3 mm from the angle of the mandible.Conclusion The safety zone for the injection is at the inferior-lateral zone and 1.4-1.5 cm below the inferior border the mandible,the needle should be inserted to a depth of approximately 1.8-1.9 cm from the skin surface.
