护理用药安全手册的制定及应用

摘　　要：目的加强对临床药品的安全管理,确保患者用药安全。方法成立用药安全管理组织,针对2011年医院41个护理单元上报用药不良事件及开展的用药安全专项检查存在的问题进行原因分析,确定护理用药安全手册制定的内容及人员分工和职责;用药安全手册内容包括各种用药安全管理组织及制度,收集临床多品多规、看似听似药品并拍成图片进行比对,将临床特殊用药药理知识以表格形式罗列;规范使用药物流程、组织培训与考核等。结果手册应用后,护士对备用药品管理规范、高危药品管理制度、专科用药知识、药品不良反应上报流程及紧急处理知晓率显著提高(均P<0.01),临床用药不良事件发生数下降。结论该手册为护士临床用药安全提供了参照资料,起到临床用药指引作用,可有效提高护理人员用药安全管理能力及自身业务水平,降低工作危机及用药风险,减少用药不良事件,保障患者用药及生命安全。Objective To strengthen management of medication safety in clinical settings, and to ensure patient medication safety. Methods A medication safety group was established. The group reviewed adverse drug events reported in 2011 by 41 units, made special inspections of drug safety, analyzed the problems found in the events and inspections, identified the contents of the medication safety manual, and gave descriptions about duties and responsibilities of personnel. The medication safety manual included such contents as medication safety regulations and policies, pictures of multi-type and multi-gauge drugs, products that seem like or sound like drugs, tables of pharmacodynamics of special drugs, standard process of medication, arrangement of training pro- grams and examinations. Results After the application of the manual, the proportions of nurses being knowledgeable with regula- tions of spare medicines, regulations of use of high risk medications, specialty-oriented medications, and with the procedure of re- porting adverse drug reaction and emergency response were significantly higher than before （P〈0.01 for all）. The incidence of ad- verse drug events had a downward trend. Conclusion The manual provides reference information about medication safety for nurses, and serves as a medication guide. The manual effectively improves nurses＇ ability to safely administer medication, prevents crisis and reduces risks in their work, minimizes adverse drug events, and ensures the safety of patients; medication and life.
