摘  要:目的:探讨腹膜外隧道法放置妇科腹腔镜手术引流管的优点,以解决引流管穿刺孔漏液的问题。方法:选择2012年8月至2013年2月施行妇科腹腔镜手术的患者160例,均于Trocar孔原位放置引流管,但放置引流管的方式不同,其中80例采用腹膜外隧道法置入(实验组),80例直接经穿刺孔置入(对照组)。对比分析两组患者拔除引流管后切口漏液情况。结果:观察组拔除引流管后切口几乎无漏液,对照组拔管后20例(25%)发生漏液,10例反复多次换药。结论:腹膜外隧道法放置引流管可有效预防拔管后切口漏液,值得推广应用。Objective:To investigate the advantages of drainage tube placement with extraperitoneal tunnel method in gyneco- logic laparoscopic surgeries for a potential solution of post-operative leakage through puncture holes. Methods:A total of 160 patients undergoing gynecologic laparoscopic surgeries from Aug. 2012 to Feb. 2013 were included in the study. Drainage tubes were placed in situ through Trocar holes. The patients were divided into two groups based on their methods of drainage tube placement. In the experi- ment group (80 patients) , drainage tubes were installed through an extraperitoneal tunnel into the abdominal cavity. In the control group ( 80 patients) , the tubes were placed directly through Trocar holes. Occurrence of leakage in the two groups was documented and com- pared after the drainage tubes were removed. Results:No wound leakage was noted in the experiment group, compared with 20 cases (25%) of leakage after tube removal in the control group, including ten patients whose dressing had to be repeatedly replaced. The difference was statistically significant ( P = 0.00). Conclusions : Placement of drainage tube through an extraperitoneal tunnel in gyne- cological laparoscopic surgery can effectively prevent postoperative leakage from the puncture hole after removal of drainage tube. This procedure is worthy of generalization.
