摘 要:目的探讨地黄饮子加减方对大脑中动脉线栓(middle cerebral artery occlusion,MCAO)模型大鼠血浆下丘脑—垂体—肾上腺轴(HPA轴)含量及其脑组织热休克蛋白70(heatshock protein 70,HSP70)表达的干预效应。方法 MCAO法制作SD大鼠脑缺血模型,造模成功后第二天开始给予地黄饮子加减方浓缩煎剂灌胃,对照组灌胃等量的生理盐水,连续7天,于最后一次灌胃后1小时取材,放射免疫法检测血浆HPA轴含量,免疫组化法检测脑组织HSP70表达。结果经地黄饮子治疗后:MCAO模型大鼠血浆皮质醇(COR)含量明显低于正常组、模型组,差异有统计学意义(P<0.01),促肾上腺皮质激素(ACTH)、促肾上腺皮质激素释放激素(CRH)含量均明显高于模型组,差异有统计学意义(P<0.05);地黄饮子组较模型组脑组织的HSP70表达更为明显,差异有统计学意义(P<0.01);模型组COR含量与脑组织HSP70表达存在显著的正相关关系,CRH、ACTH含量则与HSP70表达无明显相关性,地黄饮子组HPA轴各指标与脑组织HSP70表达无明显相关性。结论地黄饮子加减方可能通过纠正HPA轴的紊乱状态、提高大鼠脑组织HSP70表达等途径在脑缺血急性期发挥神经保护作用。Objective The aim of this study is to explore the Dihuang Yinzi Decoction's neuropro- tection effects on MCAO rats from the reaction of the HPA axis in plasma and the expression level of brain HSP70. Methods 30 SD rats were randomly divided into 3 groups: MCAO model group, herb formula group, and normal group, each group was 10. For drug treatment, herb formula group were given Dihuang Yinzi Decoction from 2days to 8days after ischemia by intragastric administration method, whereas the MCAO model group and normal group received equivalent volumes of distilled water. SD rats were killed lh after the last intragastric administration. The expression levels of ACTH, CRH, COR were detected by radioimmunoassay. The expression level of brain HSP70 was detected by Immunohistochemistry. Results The level of COR in herb formula group were significantly lower than that in normal group and MCAO model group(P 〈0. 01 ) ; The level of ACTH, CRH was significantly higher in herb formula group than in normal and MCAO model group ( P 〈 0. 05 ) ; The expression level of brain HSP70 was significantly enhanced in herb formula group than in MCAO model group (P 〈 0. 01 ) ; There was strong positive correlation between the level of COR and the expression level of brain HSP70 in MCAO model group, There were no significant negative relationships between the level of ACTH, CRH and the expression level of brain HSP70 in MCAO model group; There were no significant negative relationships between the HPA axis reaction and the ex- pression level of brain HSP70 in herb formula group. Conclusion Dihuang Yinzi Decoction may exert neuroprotection mechanism through restoring the disturbed condition of HPA axis reaction and promoting the expression level of brain HSP70.
