摘 要:目的研究P物质(Substance P,SP)、白介素6(Interleukin-6,IL-6)与腰椎间盘突出症所致腰腿痛的相关性。方法随机选取来自北京航天中心医院经临床确诊的腰椎间盘突出症26例为病例组,选取腰椎爆裂性骨折15例为对照组。于手术中获取病变节段椎间盘、患侧黄韧带和患侧小关节囊等组织,均经-80℃冰箱保存,-22℃下OCT(optim alcutting temperature)包埋冰冻切片,行SP与IL-6单克隆抗体免疫组化染色。结果病例组26例组织均可见SP及IL-6染色阳性的组织细胞,15例对照组未见相关阳性染色组织细胞。结论 SP及IL-6等炎症因子在椎间盘及邻近组织的聚集提示其参与了腰椎间盘的退变及神经根性疼痛的产生。Objective To study the relationship between substance P (SP), interleukin-6 (IL-6) with low back pain due to lumbar disc herniation. Methods All of 26 patients from Beijing aerospace center hospital with lumbar disc herniation were randomly selected as case group. And 15 patients were selected as control group, who were diagnosed as lumbar burst fracture. The lesion intervertebral disc, ipsilateral ligamentum tlavum and ipsilateral articular process joint capsule obtained during the surgery, were conserve at -80 ℃ and embedded with optimal cutting temperature (OCT) compound at -22℃. Then SP and IL-6 monoclonal antibody were used for immunohistochemical staining with frozen section. Results SP and IL-6 positive staining tissue cells were found in the case group and there was no positive staining tissue cell in the control group. Conclusion Inflammatory cytokines such as SP and IL-6, were found in the disc and the adjacent tissue, suggesting they play an important role in lumbar disc degeneration and nerve root pain.
