慢性乙型肝炎产妇脐带血HBV-DNA检测的临床意义

摘　　要：目的分析脐带血乙肝病毒DNA(HBV-DNA)定量检测在对慢性乙型肝炎产妇自发性早产及胎儿垂直感染的影响。方法 69例围产期慢性乙型肝炎产妇接受血液学及病毒学评估后,将56例患者纳入研究,分析其脐带血HBV-DNA与SPB发生的关系,并分析脐带血HBV-DNA与产妇HBV-DNA水平的相关关系,分析12个月后接种完疫苗的56例婴儿HBV感染与脐带血及母亲HBV-DNA水平的相关性。结果 56例产妇中,共有13例产妇发生SPB,SPB的发生与脐带血中HBV-DNA水平呈明显相关性(r=0.679,P<0.01)。脐带血中HBV-DNA水平与产妇HBV-DNA水平呈明显相关性(r=0.498,P<0.01)。而脐带血及产妇HBV-DNA水平并不增加婴儿乙型肝炎感染的风险。结论脐带血中HBV-DNA的表达与慢性乙型肝炎产妇SPB发生明显相关,但脐带血及产妇HBV-DNA水平并不增加婴儿乙型肝炎感染的风险。Objective To analyze the impact of quantitative determination of HBV-DNA in umbilical blood on spontaneous preterm birth（SPB） and fetal vertical infection of maternity with chronic hepatitis B. Methods After hematology and virology assessment of 69 maternities with perinatal chronic hepatitis B,56 patients were included in the study.The relationships between HBV-DNA in umbilical blood and the occurrence of SPB,the correlation of HBV- DNA level in umbilical blood and HBV-DNA level of maternities,and the correlation of HBV infection of 56 infants with vaccination vaccine after 12 months and HBV-DNA level in umbilical blood and maternities were analyzed. Results In 56 maternities,13 maternities occur SPB,the occurrence of SPB and HBV-DNA level in umbilical blood were significantly correlated（r--O.679,P 〈 0.01）.The HBV-DNA level in umbilical blood and maternities were significantly correlated（r=0.498,P 〈 0.01）.But the HBV-DNA level in umbilical blood and maternities did not increase the risk of infant hepatitis B infection. Conclusion The expression of HBV-DNA in umbilical blood and the SPB occurrence of maternity with chronic hepatitis B is significantly correlated,but the HBV-DNA level in umbilical blood and maternities don＇t increase the risk of infant hepatitis B infection.
