亚洲型股骨近端防旋髓内钉在不稳定型股骨转子间骨折中的应用

摘　　要：目的评价亚洲型股骨近端防旋髓内钉在不稳定型股骨转子间骨折中的应用情况。方法本院自2009年1月-2012年12月收治的64例股骨转子间骨折患者，通过对术中出血量、手术时间、骨折愈合时间、术后Harris评分等评定亚洲型PFNA在不稳定型股骨转子间骨折中的应用情况。结果平均手术时间（54．3±0．8）min，平均术中失血量（105．4±0．2）mL，随访时间为2～17个月，1例患者于随访时间范围因心衰死亡，无感染、下肢深静脉血栓、坠积性肺炎等并发症；60例患者末次随访均达到骨性愈合标准，无内固定失败病例；术后1年Harris评分平均为（91．3±11．4）分。结论亚洲型PFNA在治疗不稳定型股骨转子间骨折中可以取得较好效果，尤其适用于合并骨质疏松的老年患者。Objective To evaluate the applications of Asian proximal femoral nail anti-rotation in unstable intertrochanteric hip fractures. Methods We collected 64 patients diagnosed with intertrochanteric hip fractures from January 2009 to December 2012 in our hospital. Throughretrospective.appraisal of their intraoperative blood loss,operative time,fracture healing time, postoperative Harris score,we evaluated the applications of Asian proximal femoral nail anti-rotation in unstable intertrochanteric hip fractures. Results The mean operating time was （54.3 ± 0.8） min,the mean intraoperative blood loss was （105.4 ± 0.2）mL,follow-up time was 2 to 17 months and during the follow- up period one patients died because of heart failure, without other complications such as infection,thrombosis of the lower extremity andhypostatic pneumonia; at the last time of follow- up,60 cases achieved bone healing standard without internal fixation failure cases;the mean Harris score 1 year after surgery was（91.3 ±11.4）. Conclusion Asian PFNA could achieve good surgical outcome in the treatment of unstable intertrochanteric hip fractures,especially be suitable for elderly patients with osteoporosis.
