摘　　要：目的探讨3-磷酸甘油酸脱氢酶(D-3-phosphoglycerate dehydrogenase,PHGDH)抗体对自身免疫性肝炎诊断意义。方法选择在北京佑安医院诊治的自身免疫性肝炎(AIH)40例,原发性胆汁性肝硬化(PBC)患者34例,伴抗核抗体阳性(ANA≥1∶320)慢性病毒性肝炎(乙型+丙型)患者43例,免疫印迹法检测各组血清中PHGDH抗体,分析PHGDH抗体与其他自身抗体出现特点,并采用掊2检验分析AIH组患者与其他组间PHGDH抗体阳性率差别。结果共完成117例的标本检测,PHGDH抗体阳性18例,其中AIH 40例中PHGDH抗体阳性12例,阳性率30.7%;34例PBC中,PHGHD抗体阳性3例,阳性率8.8%;慢性病毒性肝炎43例中PHGDH抗体阳性3例,阳性率6.9%。两组间差异有统计学意义(P=0.026),AIH组阳性率明显高于PBC组,AIH组与病毒性肝炎组比较差异有统计学意义(P=0.007)。AIH组阳性率明显高于病毒性肝炎组;12例PHGDH抗体阳性AIH患者伴随自身抗体中ANA滴度≥1∶1000 6例,1∶320 5例,1∶100 1例,伴平滑肌抗体(SMA)阳性5例,12例阳性患者均不伴SLA/LP抗体及肝肾微粒体(LKM-1)抗体。结论 3-磷酸甘油酸脱氢酶抗体在自身免疫性肝炎患者中检出率显著高于在PBC组和病毒性肝炎组,可能有助于AIH诊断。Objective To evaluate significance of D-3-phosphoglycerate dehydrogenase （PHGDH） antibodies in diagnosis to Autoimmune hepatitis. Methods Blood serum sample were collected from 117 patients in Beijing Youan Hospital, including 40 cases with autoimmune hepatitis, 34 cases with primary biliary cirrhosis and 43 cases with chronic viral hepatitis（B and C） with （ANA 〉 1：320）. The frequency of PHGDH autoantibody in sera was determined by immunological blotting. Differentiation characteristic of PHGDH antibodies and other antibodies between AIH groups and others groups were analysed, the positive rate of PGDH antibody and other antibodies between AIH groups and others groups were analysed using chi-square test. Results Among 117 cases, 18 cases were positive for PHGDH antibodies. There are 12 cases in AIH group, with 30.7% positive rate, 3 cases in PBC group, with 8.8% positive rate and 3 cases in chronic virus hepatitis group, with 6.9% positive rate. There is significant difference between AIH groups and other groups, （P = 0.026, P = 0.007, respectively）. So the PHGDH antibodies positive rate in AIH group is higher than other groups. The character-istie of AIH patients with PHGDH antibodies positive was analysed, 12 case with ANA, 5 case with SMA, no case with SLA/LP and LKM-1. Conclution The positive rate of PHGDH antibodies was 30.7% in AIH group, and PHGDH anti bodies combining with other autoantibodies may help improve diagnosis of AIH.
