摘  要:目的研究肺癌骨转移的特点及分布规律。方法收集91例肺癌骨转移患者的多层螺旋CT(MDCT)图像资料,回顾性分析肺癌骨转移的特点及分布规律。结果 (1)91例中,腺癌48例(52.7%),鳞癌27例(29.7%),小细胞癌9例(9.9%),腺鳞癌7例(7.7%)。(2)肺癌骨转移常为多发病灶64例(70.3%),多分布于肋骨51例(56.0%)及脊柱48例(52.7%),脊柱转移以胸椎最多见。(3)60例表现为溶骨型转移(65.9%),18例表现为成骨型转移(19.8%),13例表现为混合型转移(14.3%)。结论肺腺癌较其他病理类型多发生骨转移;肺癌骨转移多分布于肋骨及脊柱;MDCT具有高空间分辨率及强大的图像后处理功能,可进一步提高诊断肺癌骨转移的准确率。Objective To study the distribution regularity and characteristics of osseous metastasis from lung cancer. Methods The image data of multi-slice spiral CT (MSCT) of 91 cases of osseous metastasis from lung cancer were collected, and the distribution regularity and characteristics of osseous metastasis were retrospectively analyzed. Results ( 1 ) Among the 91 cases, there were 48 ones of adenocarcinoma ( 52.7% ), 27 ones of squamous cancer ( 29.7% ), 9 ones of small cell carcinoma ( 9. 9% ), and 7 ones of adenosquamous carcinoma(7.7% ). (2) The osseous metastasis from lung cancer always had multiple niduses ( 64/91,70.3% ) which mainly distributed over ribs(51/91,56.0% ) and vertebral column(48/91,52.7% ). The most common site of vertebral metastasis was thoracic vertebra. (3)There were 60 cases of osteolytic metastasis (65.9%), 18 ones of osteogenic metastasis (19.8% ), and 13 ones of mixed metastasis( 14.3% ). Conclusion Lung adenocarcinoma has more osseous metastases than other pathological types. The osseous metastases from lung cancer mainly distribute over ribs and vertebral column. MSCT has advantages of high spatial resolution and great image postprocessing function. It may further improve the accuracy rate of diagnosis for bone metastases from lung cancer.
