完全经脐单孔腹腔镜胆囊切除术55例临床分析

摘　　要：目的：探讨完全经脐单孔腹腔镜胆囊切除术的疗效及应用价值。方法：回顾性分析本院55例完全经脐单孔腹腔镜切除术患者的临床资料。结果：46例手术成功,6例中转传统三孔腹腔镜,3例中转开腹胆囊切除术,手术时间为40~160 min,平均46.4 min。术后无出血、胆汁漏等并发症发生,患者恢复良好,治疗及美容效果满意。结论：经脐单孔腹腔镜切除术安全可行,但手术难度大于LC术,必须在熟练掌握传统LC的基础上进行,注意适应证的选择,其微创和美容效果明显优于传统LC,可达到腹部无可见瘢痕的效果,具有临床推广价值。Objective：To investigate the therapeutic effect and application value of totally transumbilical single-port laparoscopic cholecystectomy.Method：55 cases in our hospital were retrospectively analyzed completely by the transumbilical single-port laparoscopic cholecystectomy in patients with clinical data.Result：The operations of 46 cases were successfully completed.6 cases into traditional three holes laparoscopic surgery.3 case into open cholecystectomy.Average surgery time was 46.4 minutes（40-160 minutes）.No drainage tube was placed.No portrelated or surgical complications such as bile leak or uncontrolled hemorrhage occurred postoperatively.Patients were satisfied with the therapeutic and cosmetic effects.Conclusion：The transumbilical single-port laparoscopic cholecystectomy is safe and feasible,but more difficult than traditional laparoscopic cholecystectomy.indications should be carefully chosen,and surgeons must be familiar with the traditional laparoscopic cholecystectomy.The minimally invasive and cosmetic result is much better than the traditional laparoscopic cholecystectomy.It can achieve the purpose of no visible scars at the abdomen,and has clinical value of generalization.
