宁夏回汉族中老年男性睾酮、雌二醇水平与血脂代谢的相关性研究

摘　　要：目的通过对宁夏地区回汉族40～85岁男性血清睾酮(T)、雌二醇(E2)及血脂水平进行检测分析,探索性激素水平变化是否存在民族差异和各年龄组性激素水平与血脂的相关性。方法选取长期居住在宁夏地区40～85岁男性体检者共276例,回汉之比1∶1.76。检测T、E2与总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C),并进行分析。结果①与正常成年男性正常范围比较,T值明显下降,E2有所升高(P<0.05);②随年龄增加T值下降,E2值升高,E2/T值随年龄有升高趋势,差异均有统计学意义(P<0.05);③回汉人群中T、E2与血脂各成分差异有统计学意义(P<0.05);④T在高胆固醇血症组及LDL-C、HDL-C组较正常组显著下降,差异有统计学意义(P<0.05);⑤多元线性回归分析显示,HDL与T呈显著正相关(r=0.013,P<0.05),T与TG呈负相关(r=0.024,P<0.05),民族与T无明显相关性;⑥T值<260 ng·mL-1组较T>260 ng·mL-1组高血压及冠心病的发生率明显升高,差异有统计学意义。结论宁夏地区中老年男性人群与正常成年男性正常范围比较,T值明显下降,E2有所升高,T与男性血脂代谢紊乱相关,可能参与男性高血压及冠心病的发病。Objective To analyze the levels of testosterone, estradiol and blood lipid in male Hui and Han nationalities （aged 40 to 85 ） in Ningxia, to explore whether the variation of gonadal hormone concentrations had nationality difference and the correlation of the gonadal hormone concentrations and blood fat among different ages. Methods 276 cases of 40 to 85 year - old male permanent resi- dent who had physical examination,were randomized into our study,the ratio of Hui and Han nationality was 1 ： 1.76,testosterone （T） , estradiol （ E2 ）, total cholesterol （ TC ）, triglyeeride （ TG）, high density lipoprotein cholesterol （ HDL - C ） and low density lipoprotein cholesterol （ LDL - C ） were tested. Results T value decreased sharply while E2 increased compared to normal range of normal adult men （ P 〈 0.05 ） ; T valued decreased, E2 increased while E2/T value increased with age got older （ P 〈 0.05 ） ; There was no statistic difference of T, E2 and other content of blood fat in the two ethnic populations;T value dropped severely in hypereholesteremia and low or high - density lipoprotein groups compared to control group, which also had statistic difference ; The analysis of multiple linear regression showed that HDL had significant positive correlation with T （P 〈 0.05, r = 0.013 ） ,T had negative correlation with TG （P 〈 0.05, r = 0. 024） , nationality had no relevance with T ; Group of T value lower than 260ng/ml had high incidence rate of hypertension and coronary disease than those T higher than 260 ng~ ml - 1. Conclusion T value of Ningxia medium - elderly men decreases sharply while E2 in- creased compared to normal range of normal adult men, T is related to lipid metabolism disorder,which may involve in morbidity of hypertension and coronary disease. There is no significance difference in two ethnic populations.
