血必净联合血液灌流治疗有机磷农药中毒致多器官衰竭30例

摘　　要：目的观察血必净联合血液灌流治疗有机磷农药中毒所致多器官衰竭的临床疗效。方法将收治的有机磷农药中毒患者60例,根据治疗方案分为单纯灌流组和联合治疗组,各30例。单纯灌流组为常规急救治疗基础上应用血液灌流技术,联合治疗组在单纯灌流组的基础上联合应用血必净注射液治疗,治疗周期为1周。结果两组患者治疗后全部成活,血清丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、肌酸激酶、肌酸激酶同工酶、乳酸脱氢酶、肌酐水平显著下降,血清炎性因子C反应蛋白、丙二醛、肿瘤坏死因子α水平显著降低,超氧化物歧化酶活性显著升高(P<0.05),但联合治疗组在恢复的时间及效果明显优于对照组(P<0.05)。结论血必净联合血液灌流可降低有机磷农药对重要脏器的损害、降低炎性因子水平,有利于患者预后,提高治愈率。Objective To summarize and analyze the clinical effect of Xuebijing combined with hemoperfusion for treating multiple organ failure caused by organophosphorus pesticide poisoning. Methods 60 cases of organophosphorus pesticide poisoning admitted to our hos- pital were divided into the pure perfusion group and the combined treatment group according to the treatment schemes, 30 cases in each group. The pure perfusion group was given the hemoperfusion therapy on the basis of conventional first aid treatment; the combi- nation treatment group was combined with using Xuebijing on the basis of the simple hemoperfusion therapy. The treatment cycle was 1 week. Results All the cases survived after treatment,serum ALT and AST,CK,CKMB and LDH and Cr levels were significantly de- creased,serum inflammatory factors CRP, MDA and TNF-ct levels were reduced,the SOD activity was significantly increased,but the recovery time and effect in the combined treatment group were obviously superior to those in the control group. Conclusion Xuebijing combined with hemoperfusion can reduce the vital organs damages caused by organophosphorus pesticide, reduce the levels of inflamma- tory factors,and benefit to the prognosis and improvement of the cure rate.
