摘 要:目的探讨追踪管理法和品管圈在产科护理质量持续改进中的作用。方法成立追踪管理团队,由产科护理质量管理组、专项护理质量管理组和院级护理质量管理组组成,各级护理质量管理组依据三级评审标准制定追踪管理优先关注流程、内容或团队,并应用个案追踪和系统追踪法对产科优先关注项目或流程进行持续监测与评估,每月将所存在的问题归为10大类,并运用品管圈活动持续改进产科护理质量。结果实施1年后,产科的查对制度执行合格率显著高于实施前;患者身份识别项目不全/错误发生率、口服/静脉/肌注药物错误发生率、送临产孕妇入产房时机不准确率等终末指标显著低于实施前(P<0.05,P<0.01)。结论追踪管理法和品管圈活动完善了产科护理质量管理体系,促进了产科护理质量持续改进。Objective To investigate the role of tracer methodology and quality control circle (QCC)activities in continuous improve- ment of obstetric nursing quality. Methods We built up a tracer team, which included an obstetric nursing quality group, a special quality group, and a hospital nursing quality group. The 3 groups formulated processes, contents or clinical service groups which should be paid priority attention to, and employed individual tracking and system tracking methods for continuous monitoring and assessment of the priority aspects of obstetric nursing care. Then we categorized the existing problems into 10 aspects, and the problems of prime concern were settled through QCC activities to boost continuous improvement of obstetric quality. Results After 1 year, the rates of qualified implement of obstetric checking system were significantly higher than before; and rate of patient iden- tification failures/errors , rate of oral/intravenous/intramuscular medication errors, and rate of sending women into the delivery room at the wrong time were significantly lower than before (P〈0.05 ,P〈0.01). Conclusion Tracer methodology and QCC activi- ties help build sound obstetric nursing quality management system and enhance the quality of obstetric nursing service.
