摘  要:目的分析未足月胎膜早破新生儿的临床特点及脑电图结果,探讨脑电图异常的危险因素。方法对355例未足月胎膜早破新生儿的临床资料、脑电图结果进行回顾性分析。结果无胎膜早破组和胎膜早破组在分娩方式(P=0.000)、胎龄(P=0.001)、出生体重(P=0.015)均有统计学差异。脑电图结果有统计学差异,胎膜早破组的脑电图异常程度更高(P=0.001),脑电图异常组的胎膜早破潜伏期时间更长(P=0.047)。二分类Logistic回归分析提示,胎膜早破潜伏期、胎龄、多胎妊娠、血气pH值、5min Apgar评分是脑电图异常的独立危险因素(均P<0.01)。结论胎膜早破与早产儿脑电图异常密切相关,胎膜早破潜伏期、多胎妊娠、血气pH值、5min Apgar评分是脑电图异常的独立危险因素。Objective To review the clinical and electroencephalographic (EEG) characteristics of preterm neonates with premature rupture of membranes. Methods The clinical features and the electroencephalographic findings of 355 preterm neonates with premature rupture of membranes were retrospectively analyzed. Results There were significant differences in delivery mode (P=0.000), gestational age (P=0.001) and birth weight (P=0.015) between premature rupture group and control group. EEG abnormalities were higher in premature rupture group (P=0.001), and those with abnormal EEG had longer latency (P=0.047). Logistic regression analysis indicated that latency, gestational age, multiple pregnancy, blood PH value, 5- min Apgar score were independent risk factors for abnormal EEG. Conclusion There is a closed association between preterm premature rupture of membranes and brain damage in the neonates presenting as the abnormal EEG.
