日本血吸虫31/32kDa抗体的检测及其在诊断中的作用

摘　　要：目的 :探讨纯化日本血吸虫 31 /32kDa抗原 (Sj31 /32 )诊断的特异性、敏感性及疗效考核价值。方法 :利用纯化Sj31 /32与可溶性虫卵抗原 (SEA)ELISA方法检测急、慢性日本血吸虫病患者血清。结果 :急、慢性血吸虫病患者血清抗Sj31 /32抗体检出率为 1 0 0 %和 95% ,与正常人、肝吸虫和肺吸虫病患者血清未出现交叉反应 ;抗SEA抗体检出率为 97.4 %和 90 % ,与正常人和肠道吸虫病患者有不同程度的交叉反应。血吸虫病人治疗后 3个月、半年和 1年抗Sj31 /32抗体的阴转率分别为 52 .5%、72 .5%和 82 .5% ,而抗SEA抗体的阴转率则分别为 1 5%、37.5%和 50 %。结论 :Sj31 /32抗原诊断血吸虫病敏感性高、特异性强 ,并且具有较好的疗效考核价值 。Objective To study the specificity,sensitivity and therapy efficacy of the purified Schistosoma japonicum 31/32kDa (Sj31/32) antigen in the diagnosis for schistosomiasis japonica.Methods Schistosomiasis sera were detected with purified Sj31/32 antigen and soluble egg antigen (SEA) by ELISA.Results The positive rate of anti Sj31/32 antibody in sera from patients with acute and chronic schistosomiasis japonica were 100% and 95%, while the positive rate of anti SEA antibody were 97.4% and 90%.No cross reaction was produced by Sj31/32 antigen,while some nonspecific reaction were emerged in normal human and patients with paragonimiasis and clonorchiasis by SEA.At 3、6 and 12 months after chemotherapy,the negative conversion rate of anti Sj31/32 antibody were 52.5%、72.5% and 82.5%,while the negative rate of anti SEA antibody were 15%、37.5% and 50%.Conclusion Comparing with SEA,the Sj31/32 antigen shows higher sensitivity and specificity for the diagnosis of schistosomiasis japonica,and is of value to the evaluation of therapy efficacy.It may be used in the clinical practice and scene diagnosis of schistosomiasis japonica.
