摘　　要：依据信息经济学和制度经济学的相关理论对医疗保险总额预付制的试点改革进行了分析,认为棘轮效应的存在弱化了激励机制,引发了逆向选择和道德风险。需要进行配套措施和制度环境的改革以增强医疗保险总额预付制的制度嵌入性。Based on the theory of information economics and institutional economics, this paper analyzed the pilot reform of China＇s medical insurance total budget prepayment system and deemed that the ratchet effect would weaken incentive mechanism and led to adverse selection and moral hazard. Supporting measures and the reform of the institutional environment can enhance the embeddability of medical insurance prepaid total system.
