摘  要:目的 研究彩色多普勒超声 (ColorDopplerUltrasound以下简称CDUS)检查下肢移植血管的价值、方法、阳性指标的出现时间以及阳性率。方法 运用CDUS诊断仪检查下肢血管旁路移植术后病人 35 2例。在检查中需密切注意血流色彩、多普勒频谱及流速的改变以及血管内径的变化 ,以流速改变最为重要。根据血流速度的变化确定狭窄的程度。结果 超声诊断为血管狭窄的有 12 5例 (130处 )。其中 ,狭窄 5 0 %~ 70 %占 40 % (5 2 /130 ) ,70 %~ 90 %占 36 92 % (48/130 ) ;>90 %占 2 3 0 8% (30 /130 )。狭窄大于 90 %的与X线血管造影或手术结果对照 ,诊断敏感性 93 75 % (30 /32 ) (超声诊断 30处狭窄而X线血管造影发现 32处 ) ,假阴性 2例 ,占 6 2 5 % (2 /32 ) ,假阳性为零。同时发现狭窄的发生率随着术后时间的延长而增加 ,一年内为 16 5 3 % (2 0 /12 1) ;三年内为 2 9 41% (30 /10 2 ) ;五年内为 49 36 % (38/77) ;七年内为 70 12 % (37/5 2 )。结论 CDUS是一种下肢血管旁路手术后随访检查的有效方法。Objective To study the value and explore the scanning method of color Doppler ultrasound(COUS)in diagnosing stenosis of lower extremity bypasses.Method 352 patients were checked with CDUS. We noticed the changes of color signals and velocity of flow inside vessels The results were compared with the results of angiography or operations Results 125 vessels were showed setnosis or occlusion in one week to seven years after operations 40%(52/130)were 50~70% stenosis;36 29(48/130) were 70~90% stenosis;23 08%(30/130) were >90% stenosis Compared with angiography or operations,diagnostic sensitivity of the last group was 93 75%(30/32)(30 places were diagnosed stenoses by CDUS,But,angiography found 32 places) There were two false negative cases(6 25%)and no false positive case in this study In the same time ,we found that the conger of the time after operation,the higher of the occurrence rate In one year,occurrence rate was 16 53%(20/121);in three years was 29 41%;(30/102);in five years was 49 36%(38/77);in seven years was 70 12%(35/52) Conclusion CDUS is better way for bypass follow-up.
