TLR-4、IL-1与膝骨关节炎相关性的临床研究

摘　　要：目的检测TLR-4和IL-1β在骨关节炎(OA)患者关节液和血清中的水平,探讨其临床意义。方法应用酶联免疫吸附试验(ELISA)检测TLR-4和IL-1β在20例OA患者和20例正常人关节液和血清中的水平,分析OA患者的TLR-4和IL-1β水平与正常人水平关系,分析与关节炎严重指数的相关性。结果 OA患者血清TLR-4和IL-1β水平与正常人有统计学差异(P<0.05),且高于正常人水平。OA患者中TLR-4和IL-1β血清水平与关节液水平变化联系密切,其水平变化与关节炎严重指数呈正相关(P<0.05)。结论 TLR-4和IL-1β在OA患者中高表达,他们的表达与关节炎严重程度正相关,提示二者可能与参与OA的发生、发展过程。提示运用TLR-4和IL-1β的相关配体作为免疫佐剂可能用于治疗OA。Objective To detect the level of TLR-4 and IL-1β in synovial fluid and the serum of the patients with osteoarthritis （OA） and discuss the clinical significance. Methods 20 OA patients and 20 healthy volunteers were included in this study. The TLR-4 and IL-1β levels in synovial fluid and serum were examined by enzyme-linked immunosorbent assay （ELISA）. The data were analysed to determine the correlation between the arthritis severity index （ISOA） and the level of TLR-4 and IL-1β. Results The TLR-4 and IL-1β in the serum of the OA patients significantly higher than those of the healthy volunteers （ P 〈 0. 05 ）. There was correlation between the level of TLR-4 and IL-1β in serum and that in the synovial fluid. There was a positive correlation between the TLR-4/IL-1β changes and the ISOA （P 〈 0. 05 ）. Conclusion There is a high expression of TLR..4 and IL-1β in OA patients which is associated with ISOA. This suggests that both TLR-4 and IL-1β may be involved in the development process of OA, and ligands of TLR-4 and IL-1β may be used as an immune adjuvant for the treatment of OA.
