摘  要:目的探讨下呼吸道产超广谱β-内酰胺酶(ESBLs)大肠埃希菌和肺炎克雷伯菌的分布及耐药性,为临床合理使用抗菌药物提供依据。方法采用MicoScaWalkway40SI及纸片扩散法进行菌株鉴定及药敏试验并分析细菌分布和耐药性。结果我院下呼吸道产ESBLs大肠埃希菌和肺炎克雷伯菌2010年为89.2%和42.6%,2011年为84.6%和46.2%,这两年差异无统计学意义(P>0.05);产ESBLs对大部分抗菌药物耐药,对亚胺培南、氧哌嗪青霉素/他唑巴坦、丁胺卡那霉素、头孢西丁耐药率较低。结论大肠埃希菌和肺炎克雷伯菌产ESBLs发生率这两年无明显变化,及时监测ESBLs菌的发生率及其耐药趋势,对于指导临床抗菌药物合理应用及医院感染的控制具有重要意义。Objective To investigate the drug resistance of ESBLs-producing Escherichia coli and Klebsiella pneumoniae from specimens of lower respiratory tract infection from 2010 to 2011, and to provide clinical guidance for the rational use of antimicrobial drugs. Methods MicoScaWalkway40SI and disk diffu- sion method were used for strain identification and susceptibility testing. The bacterial distribution and drug resistance were analyzed. Results The detection rate of ESBLs-producing Escherichia coli and KlebsieUa pneumoniae in lower respiratory tract in our hospital was 89.2% and 42.6% in 2010, and 84.6% and 46.2% in 2011. The ESBLs-producing strains were resistant to most antibiotics, with low resistance to imipenem, piper- acillin/tazobactam, amikacin, cefoxitin. Conclusion The incidence of Escherichia coli and Klebsiella pneu- moniae producing ESBLs shows no significant change in the past two years. The timely monitoring of the inci- dence of ESBLs-producing strains and the drug resistance trends is of great significance in guiding the clini- cal rational use of antibiotics and the control of hospital infection.
