室性期前收缩Lorenz散点图的图形特征研究

摘　　要：目的探讨室性期前收缩Lorenz散点图的图形特征。方法选择我院2006年1月—2013年1月住院及门诊收治的室性期前收缩>400次,房性期前收缩<50次的患者174例,采用TLC4000动态心电图仪,绘制全部心搏Lorenz散点图及窦性心搏Lorenz散点图,记录期前收缩点集、期前收缩前点集、期前收缩后点集、主点集及二联律点集。结果阵发性室性期前收缩二联律呈三分布图;频发室性期前收缩伴完全代偿间期呈四分布图;频发室性期前收缩、部分期前收缩为间位性时呈五分布图。室性期前收缩Lorenz散点图期前收缩点集和期前收缩后点集位置多变,密度不同;靠近原点或远离原点;其长度可较长,≥主点集的长度。室性期前收缩的期前收缩后点集与45°线有明显的角度,为14°～21°,平均19°,其上端出现斜率不变、拐向45°线及背离45°线三种走向。结论室性期前收缩Lorenz散点图独特的图形特征可反映室性期前收缩的频率依赖性及联律间期。Objective To analyze the characteristics and clinical significance of the Lorenz plot on premature ventricu- lar contraction. Methods 174 patients admitted to our hospital from January 2006 to January 2013 with premature ventricular contraction 〉 400 and premature atrial contraction 〈 50 were selected. Dynamic electrocardiograph TLC4000 was used to draw all cardiac Lorenz scatter plot and sinus cardiac Lorenz scatter plot. The premature contraction plot, before premature contraction plot, after premature contraction plot, main plot and bigeminal rhythm plot were recorded. Results The bigeminal rhythm before paroxysmal premature ventricular contraction showed three - dense figure. Frequent premature ventricular contraction combined with complete compensation interval showed four - dense figure. Frequent Interpolated premature ventricular contraction showed five - dense figure. The premature contraction spot and after premature contraction spot of Lorenz scatter plot showed various loca- tions and densities. The plots were whether close to the origin point or far from it. The after premature contraction spot set showed an obvious angel of 14° to 21° with the 45° line, with an average angel of 19°. The top of the spot set showed consistent slope, turning to 45° line and departing from 45° line. Conclusion Lorenz plot of ventricular premature contraction can reflect the rate dependence and coupling interval of the ventricular premature contraction, which is different from atrial premature contraction.
