心肌酶测定在不同程度新生儿黄疸中的临床意义

摘　　要：目的:探讨心肌酶谱在不同程度新生儿黄疸诊治过程中的意义。方法:选取本院新生儿黄疸病例,按黄疸程度分为3组,轻度黄疸组TBIL≤220.6μmol/L(12.9mg/dl),中度黄疸组220.6μmol/L<TBIL<307.8μmol/L,重度黄疸组TBIL≥307.8μmol/L(18.1mg/dl),测定3组患儿的心肌酶及其同功酶(AST、LDH、CK、CKMB)的检查结果进行相关分析。结果:共观察新生儿黄疸160例,轻度黄疸组32例,中度黄疸组92例,重度黄疸组36例,3组病例心肌酶谱及其同功酶均有不同程度升高,其中轻度黄疸组和中度黄疸组间AST、CK有明显差异(P<0.05),LDH、CK-MB水平差异不显著(P>0.05),重度黄疸组和前两组间LDH、CK有明显差异(P<0.05),AST、CK-MB有显著差异(P<0.01)。结论:新生儿黄疸患儿均有不同程度心肌损害,尤其是重度黄疸患儿,临床诊治过程中应重视心肌酶测定及心肌保护治疗。Objective：To investigate the myocardial enzymes in varying degrees in the diagnosis and treatment of neonatal jaundice process significance.Methods： Selected hospital cases of neonatal jaundice,jaundice divided into three groups,the mild jaundice group TBIL ≤ 220.6μmol/L（12.9mg/dl）,moderate the jaundice group of 220.6μmol/LTBIL307.8μmol/Lsevere jaundice group TBIL≥307.8μmol/L（18.1mg/dl）,check the results of the determination of the 3 groups were cardiac enzymes and their isoenzymes（AST,LDH,CK,CKMB） analysis.Results： We observed 160 cases of neonatal jaundice,mild jaundice group of 32 patients,92 patients with moderate jaundice,severe jaundice group of 36 patients,three groups of cases of myocardial enzymes and their isoenzymes have different levels elevated,which light degree of jaundice and moderate jaundice group AST,CK significant difference（P0.05）,LDH,CK-MB level was not a significant difference（P0.05）,severe jaundice group and the first between the two groups of LDH,CK obvious differences（P0.05）,AST,CK-MB significant differences（P0.01）.Conclusion： Neonatal jaundice varying degree of myocardial damage,especially severe jaundice,Clinical diagnosis and treatment process should pay attention to in the determination of myocardial enzymes and myocardial protection treatment.
