活血化瘀法对结核性胸膜炎患者胸水TNF-α和TGF-β_1的干预及胸膜厚度的影响

摘　　要：目的探讨活血化瘀法对于结核性胸膜炎患者胸水中肿瘤坏死因子（TNF-α）和转化生长因子-β1（TGF-β1）的干预作用,以及对胸膜增厚的影响。方法将符合条件的60例结核性胸膜炎患者简单随机分为治疗组与对照组各30例。住院期间均予常规治疗（抗结核药物＋胸腔穿刺抽液术）,而治疗组同时加用活血化瘀的中药,疗程均为7~21 d。出院后治疗组继续口服抗结核药物＋活血化瘀中成药,对照组只给予口服抗结核药物,疗程8周。观察两组治疗前后胸水TNF-α和TGF-β1变化,以及胸膜厚度的改变。结果治疗组患者治疗后胸水中TNF-α和TGF-β1的含量明显减少,胸膜增厚的程度明显减低,与对照组相比,差异均有统计学意义（P〈0.05）。结论活血化瘀的中药能明显较少患者胸水中TNF-α和TGF-β1的含量,以及减低患者胸膜增厚的程度,改善患者症状,提高疾病预后。Objective： To explore the tuberculous pleuritis patients,who using Chinese patent drug,which had the efficiency of promoting blood circulation for removing blood stasis,by the targets of the curative effect about pleura thickening and the pretreatment effect of TNF-α and TGF-β1 on the tuberculous pleurisy.Methods： 60 eligible patients were divided randomly into two groups including the treatment group and the control group with 30 cases in each group.While in hospital,the treatment group was used the conventional treatment of western medicine（medicine for tuberculosis and the operation of taking out hydrothorax from the thoracic cavity）,combining with Chinese patent drug,which had the efficiency of promoting blood circulation for removing blood stasis.Meanwhile,the control group was used just conventional treatment of western medicine.The course of treatment was two weeks more or less seven days.After leaving hospital,the treatment group use medicine for curing tuberculosis and Chinese patent drug,which had the efficiency of promoting blood circulation for removing blood stasis,and the control group just use western medicine for curing tuberculosis.The course of treatment was eight weeks more or less seven days.The observing targets were the change of TNF-α,TGF-β1 and pleura′s thickness of Traditional Chinese Medicine.Results： The results showed a marked reduction in the hydrothorax TNF-α and TGF-β1 of the treatment group concurrently,the degree of pleural thickening decrease more statistically.Compared with the control group.There was a significant difference between the two groups（P 0.05）.Conculsion： Chinese medicine with blood activating and stasis removing can effectively reduce the content of TNF-α and TGF-β1 in the hydrothorax.At the same time,the degree of pleural thickening are markedly abated.It do not only improve the clinical symptom,but also increase the survival of the patients.
