摘　　要：目的:探讨采用局部皮瓣成形辅助VSD负压引流治疗难愈性创面的疗效及影响因素。方法:2009年9月至2013年1月郑州大学第一附属医院整形外科采用局部皮瓣成形辅助VSD负压引流所治疗的5例难愈性创面,2例放射性溃疡,3例骶尾部溃疡合并截瘫。结果:采用局部皮瓣成形辅助VSD负压引流术后,2例患者愈合;3例患者延迟愈合,其中2例伴低蛋白血症、贫血、营养不良的骶尾部溃疡合并截瘫患者,术后皮瓣下出现腔隙;1例伴贫血的放射性溃疡患者出现皮瓣远端上皮及真皮浅层坏死。结论:局部皮瓣成形辅助VSD负压引流治疗难愈性创面疗效肯定,必要时行输血、静脉输注白蛋白,营养支持,可促进创面愈合。Objective Discuss curative effect and influencing factor of local skin flap assisted the VSD in treatment of refractory wounds. Methods 5 cases of refractory wounds were treated by local skin flap assisted the VSD in plastic surgery of the first affiliated hospital of zhengzhou university Between September 2009 and January 2013,including 2 cases of radioulcer and 3 cases of sacral rear ulcer ulcer combining paraplegia. Results After local skin flap assisted the VSD,2 cases of patients were healed, 3 cases of patients were delayed union, 2 cases of sacral rear ulcer combined paraplegia with malnutrition, hypoalbuminemia, anemia,appears gap between skin flap and wound after flap plasty, epidermis and superficialdermis of distal skin flap of a a case of radioulcer with anemia were necrosis. Conclusion curative effect of local skin flap assisted the VSD in treatment of refractory wounds is certain,do intravenous albumin, blood transfusion, nutritional support when necessary, and promote wound healing.
