压膜式twin-block矫治器治疗轻中度阻塞性睡眠呼吸暂停低通气综合征的初步研究

摘　　要：目的:探讨压膜式twin-block矫治器在轻中度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)治疗中的应用和疗效。方法:为14例轻中度OSAHS患者制作压膜式twin-block矫治器,治疗前后进行Epworth嗜睡表(ESS)和魁北克睡眠问卷(QSQ)调查;并记录患者口腔颌面系统的不适感,1个月后复查多导睡眠监测(PSG)对比疗效。结果:与戴用矫治器前比较,13例患者的呼吸紊乱指数(AHI)下降了50%以上或达到正常,客观有效率为92.86%;呼吸暂停指数、最低血氧饱和度、最长呼吸暂停时间均有明显好转(P<0.01)。生活质量除情绪部分外皆有改善(P<0.01),Epworth嗜睡程度明显减轻(P<0.01)。初戴矫治器不适主要表现为唾液增多及晨起后短暂的牙齿酸胀感,矫治器半年依从率为92.86%。结论:压膜式twin-block矫治器较为舒适,患者较易接受,是治疗轻中度OSAHS的有效方法。Objective： To study the therapeutic effect of a moulage twin-block appliance in the treatment of mild and moderate ob- structive sleep apnea hypopnea syndrome （OSAHS）. Methods： 14 patients with mild or moderate OSAHS were treated by moulage twin-block appliance. Polysomnography（PSG） was performed with and without the appliance. Epworth sleepiness scale （ESS） and Quebec sleep questionnaire （QSQ） examinations were conducted before and 1 month after treatment. Discomfort of oral and maxillo- facial system of the patients was recorded. Results： 13 patients responded to the appliance therapy. Apnea-hypopnea index, longest apnea time and ESS were reduced（P 〈0.01 ）. Lowest oxygen saturation, the scores of QSQ except the domain of emotions were im- proved（P 〈 0.01 ）. Discomforts were mostly increasing saliva and mild teeth ache for short time in the morning. 13 patients persisted wearing the appliance for half a year. Conclusion： The moulage twin-block appliance is effective in the treatment of OSAHS.
