摘 要:目的检测外周血CD4+CD25+CD127low/-T细胞和CD4+CD25highCD127low/-T细胞,探讨调节性T细胞在系统性红斑狼疮发病中的作用。方法采用三色直接荧光素标记法和多参数流式细胞仪检测32例SLE患者及24名正常对照者外周血CD4+CD25+CD127low/-T细胞、CD4+CD25highCD127low/-T细胞的比例,同时检测CD4+CD25+CD127low/-T细胞的FOXP3表达水平及CD4+CD25+CD127low/-T细胞培养上清液IL-17和TGFE-β1的表达水平。结果 SLE患者外周血CD4+CD25high CD127low/-T细胞与正常对照组比较明显减少,差异有统计学意义(P<0.001);然而,CD4+CD25+CD127low/-T细胞与正常对照组比较无显著性差异(P>0.05)。CD4+CD25highCD127low/-T细胞与SLEDAI评分呈负相关(P<0.001)。CD4+CD25+CD127low/-T细胞与SLEDAI评分无相关性(P=1.000)。与正常对照相比,初发SLE患者CD4+CD25+CD127low/-T细胞培养基产生较多的IL-17(P<0.001),而TGF-β1的产生减少(P<0.001)。与正常对照相比,初发SLE患者CD4+CD25+CD127low/-T细胞的FOXP3表达减少(P<0.001)。结论初发系统性红斑狼疮患者CD4+CD25+CD127low/-T细胞可能具有效应性T细胞特点。Objective To detect Peripheral blood CD4 + CD25 + CD127low/-T cells and CD4 + CD25highCD127low/-T cells in patients with systemic lupus erythematosus,in order to reveal the role of Treg cells in the pathogenesis of SLE.Methods Peripheral blood CD4 +CD25 + CD127low/-T cells,and CD4 + CD25highCD127low/-T cells in 32 SLE patients and 24 healthy volunteers were analyzed by 3-color flow cytometry.The percentage of FOXP3 in CD4 + CD25 + CD127lOW/-Tregs and the level of IL-17/TGFβ1 in CD4 + CD25 + CD127low/-Tregs cultures were also measured.Results Our results showed that peripheral blood CD4 + CD25highCD127low/-Tregs in SLE patients were signifiantly lower than those of the control group(P 〈 0.001).However,the levels of CD4 + CD25 + CD127low/-Tregs in patients with SLE didn't show a significant change compared with control group (P 〉 0.05).The CD4 + CD25highCD127low/-Tregs were negatively correlated with systemic lupus erythematosus disease activity index (SLEDAI) (P 〈 0.001).But,there was no significant correlation between CD4 + CD25 + CD127low/-Tregs and SLEDAI(P =1.000).IL-17 levels were significantly increased in CD4 + CD25 + CD127low/-Tregs cultures in SLE patients compared with normal controls(P 〈 0.001),whereas the level of TGFβ1 was low in CD4 + CD25 + CD127low/-Tregs cultures compared with normal controls(P 〈0.001).The percentage of FOXP3 in CD4 + CD25 + CD127low/-Tregs were lower in in patients with new-onset SLE compared with normal controls (P 〈 0.001).Conclusion As a result,CD4 + CD25 + CD127low/-T regulatory cells may have the characteristics of effector T cells in patients with new-onset systemic lupus erythematosus.
