实时超声造影引导经皮肺穿刺的应用价值

摘　　要：目的:探讨实时超声造影引导经皮肺穿刺活检在诊断肺周围占位性病变中的应用价值。方法:将60例肺周围占位性病变患者随机分为两组,其中行超声造影同时穿刺活检者30例(实验组),二维超声引导穿刺者30例(对照组),比较两组穿刺的成功率及并发症发生率。结果:实验组的穿刺成功率和并发症发生率分别为100.00%(30/30)、0.00%(0/30),对照组的穿刺成功率和并发症发生率分别80.00%(24/30)、6.67%(2/30),实验组与对照组的穿刺成功率差异有统计学意义(P<0.05),而并发症发生率差异无明显统计学意义(P>0.05)。造影显示40.00%(12/30)的病变存在程度不同的坏死,且病变内出现坏死的比例随其径线的增大而增加。结论:实时超声造影引导经皮肺穿刺能明显提高肺周围占位性病变的穿刺成功率,为临床疾病诊断提供了十分重要的价值。Objective Investigate the application potential of real-time contrast-enhanced ultrasound （CEUS）-guided pereutaneous lung biopsy. Methods Sixty patients with pulmonary peripheral lesions were randomly enrolled into the experimental group （n = 30） and the control group （n = 30）. Patients in the experimental group received biopsy in CEUS, while patients in the control group received biopsy in two-dimensional ultrasound. The rate of successful biopsy and complication occurrence were analyzed. Results The rate of successful biopsy and complication occurrence were 100.00% （30/30） and 0.00% （0/30） in the experimental group, while those in the control group were 80.00% （24/30） and 6.67% （2/30）, respectively. The rate of successful biopsy in the experimental group was higher than that in the control group （P 〈 0.05）. No statistical significance was found in the complication occurrence between the two groups. The rate of lesions with necrosis was 40.00%（12/30） in the experimental group. The necrosis incidence increased dependent on the increasing of the size of the lesion. Conclusions The real-time CEUS-gnided percutaneous lung biopsy could efficiently improve the rate of biopsy success in pulmonary peripheral lesions, which may provide the importantapplication potential in the clinical diagnose of pulmonary peripheral lesion.
