工型鼻贴联合活瓣式脸贴固定法在食管癌术后患者中的应用

摘　　要：目的观察改良式留置胃管固定法的护理效果。方法将符合标准的164例患者采用随机数字法分为观察组83例,对照组81例。观察组给予改良式留置胃管固定,即工型鼻贴联合活瓣式脸贴固定法固定胃管;对照组给予传统的留置胃管固定方法,即白扁带系双套结固定法。观察比较两组的更换固定带的频率、更换固定带所需要的时间、鼻胃管移位或脱出、患者舒适度情况。结果观察组更换固定带的频率和所需时间均优于对照组的传统固定方法,差异均有统计学意义(P<0.05);患者舒适度优于传统固定法(P<0.01);鼻胃管移位或脱出、鼻部和脸部压疮发生率明显低于传统方法(P<0.01)。结论改良式留置胃管固定法能有效保持皮肤清洁完整和美观舒适,避免了因固定不牢固而导致鼻胃管脱落现象或固定不适造成的自行拔管和压疮的发生,减轻了护理工作量,使患者舒适满意。Objective To evaluate the effectiveness of improved fixation method for indwelling gastric tube. Methods Totally 164 postoperative patients with esophageal cancer were randomly divided into two groups. The modified fixation method for gastric tube （the ＂SE＂ shape nose sticker combined with valve face sticker） was applied in the experimental group,white the traditional fixation method was used in the control group. The frequency and duration of replacement of the fixation band, the incidence of nasogastrie tube dislocation and patients＇ eomfort were compared between the two groups. Results There were significant differences in the frequency and duration of replacement of fixation band between the two groups （P〈0.05）. The incidences of nasogastric tube dislocation and pressure ulcer on nose and face in the experimental group were significantly lower than the control group（P〈0.01）. Moreover,the patients in the experimental group reported higher comfort degree（P〈0.001）. Conclusion The modified gastric tube fixation method can effectively keep the skin clean,intact and comfortable. It can avoid the shift or dislocation of nasogastric tube,prevent pressure ulcer due to inadequate fixation,reduce nurse＇s workload and improve patients＇ comfort and satisfaction.
