Surgiseal外用皮肤黏合剂与皮内缝合两种甲状腺手术切口闭合方式效果的对比研究

摘　　要：目的 比较Surgiseal外用皮肤黏合剂与皮内缝合在闭合甲状腺良性疾病手术切口中的效果.方法 随机数字表法将2011年1月～2013年6月在宁波市北仑区人民医院普通外科行手术治疗的甲状腺良性结节性疾病患者84例分为Surgiseal外用皮肤黏合剂组26例和Prolene线皮内缝合组58例,分别运用Surgiseal外用皮肤黏合剂和Prolene线皮内缝合来闭合颈前手术皮肤切口,观察记录皮肤闭合时间、材料费用、术后第1天疼痛评分、切口愈合情况及术后瘢痕遗留情况.结果 皮肤黏合剂组皮肤闭合时间明显短于皮内缝合组,差异有统计学意义[（5.1±1.8）min比（13.2±2.6）min,P=0.01].Surgiseal皮肤黏合剂价格明显高于Prolene线[（609元/支）比（103元/根）,P＜0.01].两组术后第1天疼痛评分[（1.7±0.5）分比（1.7±0.9）分]、切口愈合情况（切口甲级愈合率92.3％比94.8％）及瘢痕遗留情况（术后瘢痕不明显者占92.3％比93.1％）比较,差异均无统计学意义（均P＞0.05）.结论 Surgiseal皮肤黏合剂运用于闭合甲状腺手术切口,可以达到与皮内缝合相同的美容效果,且操作时间短、易掌握、无需拆线,但费用较高,适用于临床经济宽裕患者.Objective To compare the effect of surgiseal adhesive and intradermal sutures in the closure of thyroid benign diseases incision.Methods 84 patients with thyroid benign nodal diseases from January 2011 to June 2013 in General Surgery Department of Beilun District People＇s Hospital of Ningbo City were enrolled in our study,and were randomly divided into Surgiseal adhesive group （26 cases） and intradermal sutures group （58 cases）,the incisions were closed by surgiseal adhesive and intradermal sutures respectively.The skin closure time,material cost,pain score of first day after operation,incision condition and postoperative legacy scar were recorded and compared.Results The skin closure time of surgiseal adhesive group was significantly shorter than intradermal sutures group,the difference was statistically significant [（5.1±1.8） min vs （13.2±2.6） min,P =0.01].The cost of Surgiseal adhesive was significantly higher than Prolene （609 RMB vs 103 RMB,P ＜ 0.01）.There were no difference between the two groups in pain score of first day after operation [pain score （1.7±0.5） points vs （1.7±0.9） points],incision condition （rate of class A healing 92.3％ vs 94.8％）,and postoperative legacy scar （rate of no obvious postoperative legacy scar 92.3％ vs 93.1％）,the differences were not statistically significant （P ＞ 0.05）.Conclusion Surgiseal adhesive provides advantage in shorter closure time,easier management and without taking out the stitches and could achieve the same cosmetic effect as intradermal suture,but more expensive,so it would be suitable for rich patients.
