摘  要:目的牙周病、外伤都会造成患者牙槽骨缺损,严重影响患者的健康。组织工程技术的应用可重建缺失的牙槽骨,恢复其形态和功能。文中探讨应用富血小板血浆(platelet-rich plasma,PRP)与骨髓基质细胞(bone marrow stromal cells,BMSCs)构建引导骨再生膜的方法,以及应用引导骨再生膜修复牙槽骨缺损的方法。方法将取自同一Beagle犬的BMSCs与PRP复合后,共同在体外培养,构建出BMSCs膜片,将其植入供体犬的牙槽骨缺损区,另以单纯牙龈翻瓣组和单纯引导骨再生(guided bone regeneration,GBR)组作对照。术后12周取犬牙槽骨,通过大体观察、X线片观察和组织学观察分析评价其修复牙槽骨缺损的情况。结果 BMSCs与PRP共同在体外培养3周,可构建出厚约0.2 mm的BMSCs膜片。将其植入Beagle犬牙槽骨缺损区12周,在骨缺损区有大量的新生骨质形成,其成骨的量明显大于单纯牙龈翻瓣组,优于单纯GBR组。结论将PRP与BMSCs复合后共同在体外培养,可构建出具有一定厚度的BMSCs膜片,将此膜片植入牙槽骨缺损区显示出此种膜片具有良好的修复骨缺损能力。Objective Periodontitis and trauma might cause serious alveolar bone defects and damage the patients' health. Tissue engineering technology can be applied to the reconstruction of the defects of the alveolar bone and restore its shape and function. The present study was to search for a method of constructing guided bone regeneration (GBR) membranes by combining platelet-rich plasma (PRP) and bone marrow stromal cells (BMSCs) , and to use these GBR membranes in repairing the alveolar bone defects. Methods About 5 ~ l06 BMSCs were suspended in 200 p.1 of PRP, and the mixture was seeded and cultured on a plate. BMSC-sheets were harvested after three weeks of in vitro culture and transplanted to the defected alveolar bone. The control groups received flap op- eration or GBR alone. The renascent alveolar bone formation was evaluated by gross observation, X-ray, histological analysis and histo- morphometry 12 weeks after operation. Results BMSC sheets about 0.2 mm thick were successfully constructed after 3-week in vitro cuhure of BMSCs with PRP. Twelve weeks after the implantation, a mass of new bone formation was observed in the defected area of the alveolar bone, more obvious in the GBR membrane group than in the groups of flap operation and GBR alone. Conclusion Sick BMSC sheets can be constructed by in vitro culture of BMSCs with PRP and transplanted to the defected alveolar bone, which shows a good effect in bone defect repair.
