摘 要:目的:比较不同脉压的原发性高血压患者肾动脉阻力指数(RI),探讨脉压与肾动脉阻力的相关性。方法:根据24 h动态血压监测结果将入选病例分为高脉压组(19例)和非高脉压组(36例)。检测两组患者的空腹血糖、血脂和肌酐。应用彩色多普勒超声肾血流测定法测定两组患者左、右侧主肾动脉(MRA)、叶段动脉(SRA)、叶间动脉(IRA)的收缩期峰值流速(Vs)、舒张期末血流速度(Vd),计算RI。结果:高脉压组患者各级肾动脉RI均大于非高脉压组,差异有统计学意义(P<0.01)。各级肾动脉RI与脉压呈正相关(P<0.05)。结论:肾动脉RI与脉压密切相关,脉压增高可引起肾动脉阻力增加。Objective: To explore the correlation between pulse pressure and renal arterial resistance bycomparing renal arterial resistive index (RI) of hypertension patients with different pulse pressure. Methods: The selected patients were divided into two groups: the high pulse pressure group (19 cases) and the low group (36 cases) based on the 24-hour ambulate blood pressure monitoring profiles. Blood glucose, plasma lipids and creatinine were determined. Color Doppler ultrasound was applied to determine the renal blood flow including peak velocity in systole (Vs), end-diastolic velocity (Vd) of main renal artery (MRA), segmental renal artery (SRA) and interlobar renal artery (IRA). Renal arterial resistence index was calculated. Results: Patients in the high pulse pressure group had higher RI than whom in the low group. The difference was statistically significant (P〈0.01). There were positive correlation between the renal arterial RI and the pulse pressure (P〈0.05). Conclusion: Renal arterial RI has intimate related to pulse pressure. Hypertension with high pulse pressure will increase renal arterial resistance.
