摘 要:目的探讨宁夏地区多囊卵巢综合征(PCOS)患者高雄激素血症对脂代谢的影响。方法收集PCOS患者267例,其中回族89例,汉族178例,分析高雄激素临床特征。根据血清睾酮水平,将PCOS患者分为高雄激素组和雄激素正常组,比较不同雄激素状态的脂代谢情况。结果①PCOS患者总体、回族、汉族痤疮发生率分别为20.60%、16.85%、22.47%。多毛发生率为25.09%、21.35%、26.97%;高雄激素血症发生率为47.56%、50.56%、46.06%。汉族多毛发生率高于回族(P<0.01)。②在总体PCOS患者中,高雄激素组总胆固醇(CHO)、载脂蛋白B(ApoB)高于雄激素正常组(P<0.05);在汉族PCOS患者中,高雄激素组CHO、低密度脂蛋白(LDL)、ApoB高于雄激素正常组(P<0.05或P<0.01)。在回族PCOS患者中,两组各项指标比较差异无统计学意义(P>0.05)。③在IR正常的非肥胖汉族PCOS患者中,高雄激素组CHO、ApoB高于雄激素正常组(P<0.05)。结论独立于肥胖、胰岛素抵抗因素,高雄激素血症是PCOS患者脂代谢紊乱的危险因素,且回、汉族间存在差异。Objective To explore the effect of hyperandrogenemia on lipid metabolism in patients with polycystic ovary syndrome (PCOS) in Ningxia Hui Autonomous Region. Methods A total of 267 female patients with PCOS ( 178 Han and 89 Hui nationalities) were collected in this study, who were divided into high androgen group and normal an- drogen group according to their blood testosterone levels. And then their clinical characteristics of hyperandrogenemia were analyzed and compared. Results (~) In all ethnic patients, in Hui and Han patients, the incidence of acne was 20.60%, 16.85% and 22.47% ; the incidence of hirsutism was 25.09%, 21.35% and 26.97% ; the incidence of hy- perandrogenemia was 47.56%, 50.56% and 46.06%, respectively. Hirsutism was more common in Han group than in Hui group ( P 〈 0.01 ). (~) In all ethnic patients, the total cholesterol (CHO) and apolipoprotein ( ApoB ) were signifi- cantly higher in the hyperandrogenemia group than in normal androgen group (P 〈 0.05 ). In Han patients, CHO, low- density lipoprotein (LDL) and ApoB were significantly higher in the hyperandrogenemia group than in normal androgen group (P 〈0.05 or P 〈0.0l). In Hui patients, there was no significant difference between the two groups (P 〉0.05). (~) In the non-obesity Han group with non-IR, CHO and ApoB were significantly higher in the hyperandrogenemia group than in normal androgen group ( P 〈 0.05 ). Conclusion Hyperandrogenemia is the risk factor for dyslipidemia in women with PCOS, independent of obesity and insulin resistance. However, there exist differences between Hui and Han groups.
