摘　　要：目的探讨CD19+CD24hi CD38hi B细胞亚群在系统性红斑狼疮(SLE)患者外周血中的表达及临床意义。方法流式细胞术检测SLE患者(S组,36例)和正常对照组(C组,20例)CD19+CD24hi CD38hi、CD19+CD24int CD38int、CD19+CD24hi CD38-B细胞亚群百分比水平。S组中,20例为SLE活动(S1组),16例为SLE稳定(S2组)。结果 S组CD19+CD24hi CD38hi B细胞亚群百分比显著高于C组[(10.93±6.30)%vs.(7.51±2.73)%](P<0.05),且S1组高于S2组(P<0.05)。S组CD19+CD24hi CD38-B细胞亚群百分比显著低于C组[(15.52±9.54)%vs.(26.46±11.07)%](P<0.05)。S组CD19+CD24hi CD38hi B细胞亚群表达水平与SLE疾病活动指数(SLEDAI)积分正相关(P<0.05)。结论 CD19+CD24hi CD38hi B细胞亚群对效应T细胞分泌促炎症因子缺乏抑制功能与其存在功能缺陷有关,而非因其数量不足。CD19+CD24hi CD38hi B细胞亚群在SLE患者外周血中的表达水平可能与疾病活动相关。Objective To investigate the expression and clinical significance of CD19＋ CD24h＋ CD38hi B cells in the patients with systemic lupus erythematosus（SLE）. Methods CD19＋ CD24hi CD38hi ,CD19＋ CD24int CD38int and CD19＋ CD24 CD38- B cells subsets in peripheral blood were detected with flow cytometery in 36 SLE patients（group S） and 20 healthy persons（group C）. Of group S,20 cases were SLE active（group $1） and 16 cases were SLE stable（group $2）. Results The percentage of CD19＋CD24ul CD38va B cells was higher in group S than that in group C[（ 10. 93---4- 6.30）~ vs. （7.51 ＋ 2.73）~] （P〈~0. 05）, which was higher in group S1 than that in group $2 （P^0. 05）. The percentage of CD19＋ CD24h^CD38 B cells was lower in group S than that in group C [（15.52~9.54）~ vs. （26.46~11.07）%]（P%0. 05）. The expression of CD19＋ CD24h^CD38h~ B cells was positively correlated with SLE disease activity index （P-~0. 05 ）. Conclusion The lack of depressive effect of CD19＋ CD24 CD38hi B cells on proinflammatory cytokines is secondary to its functional defects rather than numerical deficiency. The expression of CD19＋ CD24~CD38~ B cells may play a role in the disease activity of SIZE.
