摘　　要：目的：研究正常生育力（无避孕同居生活12个月内使配偶获得妊娠）男性精液质量水平，并分析到达妊娠时间（nP）与精液参数相关性。方法：招募年龄在18—50岁之间，妊娠到达时间12个月内的男性，填写问卷、体检和精液分析，计算精液参数正常参考值范围，探讨TTP和其他生物学特征对精液质量的影响。结果：1152例年龄范围18—50岁的男性接受精液检查，精液体积（m1）、精子浓度（10-6／m1）、总精子数（10-6）、活动率f（PR＋Np）％]、前向运动精子[PR（％）]、正常形态精子百分率（％）、精子存活率（％）、前向运动精子总数（10-6）第5个百分位数值分别为1．5、17、39、41、33、5、53和17。年龄I〉35岁组前向运动精子总数较少（P〈0．01）；高体质量指数（BMI〉25．0）组正常形态精子百分率较低（P〈0．01），低体质量指数（BMI〈18．5）组精子浓度、总精子数、正常形态精子百分率和前向运动精子总数较低（P〈0．05）；吸烟降低精液体积（P〈0．01）；饮酒超过10∥周减少精液体积、总精子数、正常形态精子百分率和前向运动精子总数（P〈0．05）；TTP甲与精子浓度、总精子数和前向运动精子总数呈负相关（P〈0．01）；对象禁欲时间5—7d组的精子浓度、总精子数和前向运动精子总数高于其他组（P〈0．01）。结论：正常生育力男性精液参数第5个百分位数值与世界卫生组织的正常参考值范围基本相同；TTP数值与精液质量呈负相关。年龄、体质量指数和生活方式会对部分精液参数产生影响。Objective： To study the semen quality of confirmed fertile men with the time to pregnancy （TIP） of 12 months or less, and analyze the correlation of semen parameters with TIP. Methods： We recruited 1 152 men aged 18 -50 years and with TIP of 12 months. We used questionnaire investigation, physical examination and semen analysis, calculated the normal reference ranges of semen parameters, and analyzed the effects of TIP and other biological characteristics on semen quality. Results ： Seminal examina- tions showed that the semen volume was 1.5 ml, sperm concentration 17 million per ml, total sperm number 39 million per ejaculate, total sperm motility 41%, progressive sperm motility 33%, normal sperm morphology 5.0%, sperm vitality 53%, and total number of progressively motile sperm 17 million per ejaculate in the 1 152 subjects. In comparison, the total number of progressively motile sperm was significantly smaller in those aged ≥ 35 years （P 〈 0.01 ）, morphologically normal sperm were markedly less in those with BMI 〉 25.0 （P 〈 0.01 ）, and sperm concentration, total number of sperm, morphologically normal sperm rate and total number of progres- sively motile sperm were remarkably lower in those with BMI 〈 18.5 （ P 〈 0.05 ）. Cigarette smoking significantly reduced the sperm volume （P 〈0.01 ）, and drinking （ 〉 10 g per week） decreased the sperm volume and the total numbers of sperm, morphologically normal sperm and progressively motile perm （ P 〈 0.05 ）. TIP was negatively correlated with sperm concentration, total number of sperm and total number of progressively motile sperm （ P 〈 0.01 ）. The subjects with sexual abstinence of 5 - 7 days showed markedlyhigher sperm concentration, total number of sperm and total number of progressively motile sperm than others （P 〈 O. O1 ）.
