膝关节腔注射透明质酸钠结合运动疗法治疗膝骨关节炎的临床研究

摘　　要：目的:观察关节腔内注射透明质酸钠结合运动疗法治疗膝骨性关节炎的临床疗效并对疗效进行评价。方法:将90例膝骨关节炎患者随机分成治疗组及对照组(非甾体抗炎药物组),两组均治疗30d,以总有效率、WOMAC量表评分和Irrgang量表评分为观察指标。结果:总有效率:两组间治疗前后比较差异无统计学意义(P>0.05)。WOMAC量表评分和Irrgang量表评分:两组内治疗前后总分评分、两组间治疗前后总分、两组间治疗前后疼痛、症状评分比较差异均无统计学意义(P>0.05);但两组间治疗前后僵硬、躯体功能、生活功能评分比较差异均有统计学意义(P<0.05),治疗组优于对照组。结论:膝关节腔内注射透明质酸钠结合运动疗法治疗膝骨关节炎安全、有效,可以改善患者疼痛症状,且对僵硬、躯体功能生活功能的改善较药物组明显。WOMAC量表和Irrgang量表可作为临床治疗膝关节骨关节炎疗效评价的有效量化指标。Objective：To observe the clinical curative effect of knee injection with sodium hyaluronate and exercise therapy on knee osteoarthritis and assess the results. Methods： 90 patients with knee osteoarthritis were randomly divided into treatment group and control group （group of non-steroidal anti-inflammatory drugs）, both group were treated for 30 days, the total effective rate, WOMAC scale score and Irrgang scale score as observation. Results：Total effective rate： there was no significant difference between the two groups before and after treatment （P〉0.05）. WOMAC questionnaire score and Irrgang rating scale： total score before and after the treatment in two groups, scores between the two groups before and after treatment, pain and symptom scores before and after treatment between the two groups had no significant difference （P〉0.05）. But there was significant difference before and after treatment between two groups in stiffness, physical function, functional score （P〈0.05）, treatment group was superior to control group. Conclusion：Knee joint cavity injection of sodium hyaluronate combined with exercise therapy for knee osteoarthritis is safe, ideal and effective, can improve the patients pain symptoms, and on the improvement of the stiffness, the Physical function and life functions significantly better than the drug group. WOMAC questionnaire and Irrgang scale can be used as the effective evaluation indicators in the clinical treatment of knee osteoarthritis.
