摘  要:目的:观察严重烧伤大鼠肠黏膜zonula occludens-1(ZO-1)mRNA的表达和细菌易位,探讨严重烧伤时肠黏膜屏障功能的改变。方法:将50只大鼠随机分为对照组(n=20)和烧伤组(n=30)。对照组大鼠不作任何处理;烧伤组大鼠于背部造成30%总体表面积(TBSA)Ⅲ度烫伤(以下称烧伤)创面,伤后立即经腹腔补充乳酸林格液40 ml/kg。大鼠烧伤后24、48、72和96 h,取肠系膜淋巴结、肝、脾组织,匀浆后行细菌培养。采用逆转录-聚合酶链反应(RT-PCR)法检测空肠组织中ZO-1 mRNA表达水平。光镜下观察空肠黏膜组织的病理形态学变化。结果:在伤后72 h,肠系膜淋巴结、肝、脾组织匀浆细菌易位量烧伤组明显高于对照组;ZO-1 mRNA的表达水平对照组为0.854±0.0132,烧伤组为0.487±0.0153;空肠黏膜组织病理形态学观察,烧伤组肠黏膜绒毛数量减少,排列紊乱,绒毛顶端上皮细胞出现坏死和脱落。结论:严重烧伤大鼠早期存在肠黏膜屏障功能损伤,肠上皮细胞间连接增宽,其可能机制与肠上皮ZO-1 mRNA的表达降低有关。Objective: To observe zonula occludens-1 (ZO-1)mRNA expressions of jejunum tissuesand the translocation of intestinal bacteria in rats with severe burns, and investigate the relationship between ZO-1 mRNA expression and intestinal mucosal barrier injury. Methods : The rats were random- ly divided into control group ( rt = 20 ) and burn group ( n = 30, with 30% TBSA full-thickness burn on the back). The rats in the group of burn injury were resuscitated with intraperitoneal isotonic saline after burned. At 24, 48, 72 and 96 hours after burned, the mesenteric lymphatic nodes, liver and spleen were homogenized to culture for bacteria, the zonula occludens-1 ( ZO-1 ) mRNA expressions of jejunum tissues were detected by RT-PCR, the sections of jejunum tissues were routinely prepared and examined. Re- suits: Bacterial translocation quantity in MLN, liver and spleen in control group were lower than in burn group. ZO-1 mRNA was 0. 854 :1: 0. 0132 in control group and 0. 487 -0.0153 in burn group. Histologi- cal examination showed that the pathomorphologic injury in the intestinal mucosa was slighter than those in burn group. Conclusion : Intestinal mucosal barrier injury and widening of the junctions of intestinal epithelial cells occur in early stage of rats with severe burns, and the decreased expression level of ZO-1 mRNA may be involved in the possible mechanism.
