摘  要:目的了解云南省HBV基因型分布情况和其特殊性。方法对来自云南省7个地州市的55例HBV感染者,采集、分离其血清样本,核酸提取,套式PCR扩增前S1/前S2/部分S基因区段(877bp),进行核酸序列测定与同源关系和重组位点分析。计量资料比较采用t检验,计数资料比较采用卡方检验。结果建立更为准确的HBV基因分型方法,49份样本获得有效分型,13份(26.5%)为B2亚型;33份(67.3%)为C型,其中C1亚型7份,C2亚型19份,C5亚型3份,c7亚型4份;另有2份(4.1%)为I基因型和1份为(2.0%)C/D重组型。B、C型感染者HBVDNA分别为(7.69±0.86)、(6.88±1.24)lg拷贝/mL,差异有统计学意义(t=1.621,P=0.016);B、C型感染者碱性磷酸酶分别为(78.6±28.8)、(124.O±46.8)U/L,差异有统计学意义(t=-1.534,P=0.028)。结论建立的HBV基因型/亚型分析方法可准确分型;云南地区HBV多基因型/亚型分布状况,可能为东南亚地区所流行HBV的传人所致,并可能影响中国HBV基因型的分布态势。Objective The aim of this study was to describe the distribution of hapatitis B virus (HBV) genotypes and their characteristics in Yunnan Province. Methods Serum samples were collected from 55 cases of HBV infected individuals distributed in 7 cities and districts of Yunnan Province. After DNA extraction, The HBV S segment was amplified with nested polymerase chain reaction (PCR) and subsequently sequenced. Phylogenetic and recombinant analyses were performed for the obtained sequences. The data were analyzed by Chi-square test and t-test. Results A new phylogenetic analysis approach was established to make more accurate classification of HBV genotypes/subtypes. By this method, 49 samples were successfully genotyped. According to their phylogenetic relationship, 13 (26. 5%), 33 (67. 3%), 2(4.1%) and 1 (2. 0%) of samples were classified as genotype B, C, I and C/D hybrid, respectively. HBV DNA level of genotype B infected patients was (7. 69 ± 0. 86) copy/mL, which was significantly higher than that of the genotype C infected patients [(6. 88±1.24) copy/mL; t=1. 621, P=0.016]. The serum alkaline phospbatase (ALP) level of genotype C infected patients was (78. 6±28.8) U/L, which was significantly lower than that of genotype B infected patients [(124. 0±46. 8) U/L; t=-1. 534, P=0. 0281. Conclusions The newly established method for HBV classification is more accurate for HBV genotyping. The HBV genotype distribution in Yunnan is possibly impacted by the HBV prevalence in Southeastern Asia, which may become a potential influential factor on the distribution of HBV genotypes in China.
