外科手术切口感染的干预措施

摘　　要：目的探讨外科手术切口感染的危险因素,并制定对应的干预措施,从而降低感染发生率。方法回顾性分析2009年1月-2013年1月行外科手术的596例患者临床资料,采用单因素及多因素非条件logistic回归分析,模型筛选采用Stepwise法,研究外科手术切口感染的危险因素。结果 596例手术患者中共发生感染12例,感染率为2.0%;单因素分析显示,医院感染的危险因素包括高龄(>50岁)、合并外伤及基础疾病、急诊手术、手术时间长(>4h)、存在感染病灶、住院时间长(>15d)等,而多因素分析进一步表明,存在感染病灶的危险性最大(OR=3.475),其余依次为合并外伤及基础疾病(OR=3.187)、急诊手术(OR=2.976)、手术时间长(OR=2.418)、高龄(OR=2.065)、住院时间长(OR=1.871)。结论外科手术切口感染与多种因素关系密切,临床应采取对应的干预措施,如严格无菌操作、积极治疗原发病等,使感染得到有效控制。OBJECTIVE To study the risk factors of surgical wound infections in operating rooms, develop the corresponding through intervention measures so as to reduce the incidence of infections. METHODS A retrospective analysis was carried out for 596 patients who accepted the surgical treatment enrolled in the hospital from Jan 2009 to Jan 2013. The risk factors of surgical wound infections in operating rooms were studied through the univariate analysis and the multivariate logistic regress analysis, the model screening was performed with the use of Stepwise methold. RESULTS 12 cases of 596 patients had infections in the operation with an infection rate of 2.0% ;the univariate analysis showed that the risk factors of nosocomial infections were the advanced age （〉50 years old）, complication of trauma and basic diseases, emergency surgery, long operation duration （〉4 h）, infected lesions, and long length of stay （〉15 d）, etc. ; the multivariate analysis further revealed that, the risks in a descending order were respectively the infected lesions （OR = 3. 475）, complication of trauma and basic diseases （OR = 3. 187）, emergency surgery （OR=2. 976）, long operation duration （OR=2. 418）, age （OR=2. 065）and long length of stay （OR=1. 871）. CONCLUSION The surgical incision infections in the operating rooms are closely related to a number of factors. The corresponding intervention measures such as strict aseptic operation, active treatment of the primary diseases should be taken in clinicsto, so as to effectively control the infections.
