摘　　要：目的探讨ACS患者PCI术后血清sCD40L及MPO水平的变化以及血小板ADP受体拮抗氯吡格雷(波立维)对其的影响。方法急性冠脉综合征患者190例,随机分为高剂量氯吡格雷试验组(96例)和常规剂量对照组(94例)。服药前、PCI术前、术后1 d、术后7 d检测血清中MPO和sCD40L的水平。结果①PCI术后血清sCD40L、MPO水平进一步升高。②PCI术后高剂量组血清sCD40L水平整体情况明显低于标准剂量组,且P值均<0.05,有显著性差异。③PCI术后高剂量组血清MPO水平有低于常规剂量组的趋势,但P>0.05,无显著性差异。MPO与sCD40L的变化趋势的相关性检验:P>0.05,无相关性,皮尔森相关系数R=0.131。结论 PCI术后患者血清炎性因子sCD40L、MPO进一步升高;氯吡格雷对ACS患者PCI后血清sCD40L有明显的抑制作用,对MPO有减低的趋势。[ Objective ] To discuss the influence of High loading dose of Clopidogrel on serum-sCD40L and MPO levels in acute coronary syndrome patients after intervention operation. [Methods] 190 patients with acute coronary syndrome were random divide into two groups： The number of the patients of the high loading dose of Clopidogrel group was 96. The number of the patients of the standard dose of Clopidogrel group was 94. All patients have been detected serum sCD40L and MPO levels before taking Clopidogrel, at preoperative PCI, postoperative PCI for the first day, for the seventh day. [Results] The serum levels of sCD40L and MPO increased significantly after PCI. The serum levels of sCD40L with the high dose group were significantly lower than those of the standard dose group on the overall situation （P 〈0.05）, and the serum levels of MPO of high does group tend to lower than those of the stan- dard does group on the overall situation while the tendency make no sense on statistical significance. It proved that there is no relationship between MPO and the changing tendency of.sCD40L （relativity examination, P 〉0.05; Pear- son correlative coefficient, R =0.131）. [Conclusions] The serum levels of sCD40L and MPO after PCI have in- creased significantly, and this change have statistical significance（P 〈0.05）. Clopidogrel can significantly restrain the serum levels of sCD40L for patients with acute coronary syndrome （ACS） after PCI, and have the tendency to reduce the serum levels of MPO.
