大剂量氨溴索在重症肺炎集束化治疗中的应用探讨

摘　　要：目的探讨大剂量氨溴索在重症肺炎集束化治疗中的应用价值。方法将45例重症肺炎患者随机分为对照组（n=20）和治疗组（n=25）。对照组在给予氧疗、抗感染、营养支持及维持内环境稳定等集束化治疗基础上加用注射用盐酸氨溴索90mg／d静脉滴注；治疗组在集束化治疗基础上加用注射用盐酸氨溴索90mg，每6小时1次，静脉滴注。氨溴索治疗疗程为14d。比较两组患者在治疗前及治疗后第7、14天血C-反应蛋白、降钙素原、氧合指数的变化，并进行统计学处理。结果两组患者在治疗后第7、14天血C-反应蛋白、降钙素原、氧合指数等方面比较差异均有统计学意义（P〈0．05）。结论大剂量氨溴索在重症肺炎集束化治疗中能有效改善氧合，协助抗炎、抗感染作用，可以有效提高重症肺炎的治疗效果。Objective To investigate the value of large doses of ambroxol on the bundle treat-ment of severe pneumonia. Methods Forty-five patients with severe pneumonia were randomly divided into control group （ n = 20） and treatment group （ n = 25 ）. Patients in control group were given the bun-dle treatment of oxygen therapy, anti-infection, nutritional support and maintain environmental stability, and intravnous injection of ambroxol 90 mg/d. Patients in treatment group were given the bundle treat-ment and intravenous injection of ambroxol 90 mg, every 6 hours. The course of ambroxol treatment regi-mens was ld d. The blood C-reactive protein, procalcitonin, oxygenation index changes of the two groups, before treatment, on the 7th and 14th day after treatment were compared, and statistically ana-lyzed. Results There were significant differences in the levels of C-reactive protein, procaleitonin, oxy-genation index of the two groups on the 7th and 14th day after treatment （ P 〈 0.05 ）. Conclusions Large doses of ambroxol in the bundle treatment of severe pneumonia can effectively improve oxygenation, assist the anti-inflammatory, anti-infective effects, and can effectively improve the therapeutic effect of severe pneumonia.
