霉酚酸酯对糖尿病大鼠肾组织T细胞浸润的影响

摘  要:目的 探讨霉酚酸酯(MMF)对链脲佐菌素(STZ)诱导的糖尿病大鼠T淋巴细胞在肾脏组织浸润的影响.方法 30只雄性Wistar大鼠(鼠龄2~3个月),随机分为正常对照组、糖尿病模型组和MMF治疗组[15mg·(kg·d)-1].观察16周后,检测大鼠血糖(BG)、血尿素氮(BUN)、血肌酐(Scr)、肾肥大指数(肾重KW/体重BW)和24h尿蛋白定量(24Upro).用流式细胞仪方法检测肾组织中CD3+、CD4+T淋巴细胞以及细胞因子TNF-α、IFN-y的含量,免疫组化法测肾组织中CD4+、CD8+T细胞的表达.结果 与对照组相比,糖尿病(DM)组大鼠BG、BUN、Scr、Kw/Bw、24Upro等指标均显著升高,差异有统计学意义(P<0.01).通过流式细胞仪检测,糖尿病大鼠肾脏内CD3+、CD4+T细胞以及由CD4+T细胞产生IFN-γ、TNF-α均显著上调(P<0.01),通过免疫组化肾脏内CD4+、CD8+T细胞均显著升高(P<0.01).除血糖、肾肥大指数外,治疗组上述指标均比糖尿病组低,差异有统计学意义(P<0.05).结论 霉酚酸酯通过抑制T淋巴细胞在糖尿病大鼠肾脏组织中的浸润从而下调炎症因子的表达发挥肾脏保护作用.Objectives To investigate the effect of mycophenolate mofetil (MMF) on T lymphocytes infiltration of renal tissues in streptozotocin (STZ)-induced diabetic rats.Methods Twenty one male wistar rats were randomly divided into three groups:normal control group (NC group),diabetic model group(DM group) and MMF -treated group(MMF group).In 16 weeks,blood glucose (BG),blood urea nitrogen (BUN),serum creatinine (Scr),renal hypertrophy index (kidney weight/body weight,KW/BW) and 24 hour urinary protein (24Upro) were detected.The number of CD3 +/CD4 + T cells and expression of TNF-α/IFN-γ in renal tissues were measured through flow cytometry.Moreover,CD3 +/CD8 +T cells were determined by immunohistochemistry.Results Compared with NC group,BG,BUN,Scr,Kw/Bw,24 upro were significantly increased in DM group (P < 0.01),intrarenal CD3 +/CD4 + T cells and TNF-α/IFN-γ produced by CD4 + T cells were both significantly up-regulated in flow cytometry(P <0.01).Similarly,the levels of CD3 + and CD8 + T cells were elevated in kidney biopsies evaluated by immunohistochemistry(P <0.01).Except the BG and Kw/Bw,the above mentioned parameters were all inhibited in MMF group compared with DM group(P < 0.05).Conclusions MMF has an protective effect on diabetic nephropathy.The mechanisms may be explained by the inhibition of T lymphocytes infiltration and down -regulation of expression of inflammation factors in diabetic rats.
