山西省农村慢性病疾病经济负担与风险研究

摘　　要：目的:了解山西省农村居民常见慢性病的疾病经济负担与经济风险。方法:通过问卷调查,了解山西省农村地区慢性病患者最近一年疾病经济负担和经济风险现状。结果:高血压患者年人均纯收入为3 692.34元,人均年治疗费用为3 716.29元,其校正RR值为11.23;糖尿病患者年人均纯收入为4 497.63元,人均年治疗费用为4 297.95元,校正RR值为10.61;慢性支气管炎患者年人均收入为3 578.90元,人均年治疗费用为2 919.01元,校正RR值为9.10;三种疾病患者中分别只有32.41%、38.71%和24.29%的人对医疗费用有20%以上支付能力,支付能力很低。结论:山西省农村慢性病人群的疾病经济负担和风险较高,糖尿病患者的经济负担和风险要高于高血压和慢性病支气管炎患者;慢性病患者对医疗费用的支付能力很低,且有贫富两极分化的趋势。Objective： To study economic burden and risk of common chronic diseases in rural area of Shanxi. Methods： Chronic disease patients were randomly selected and investigated for their economics burden and risk of diseases through questionnaires. Results： The average annual income of hypertension patients is RMB 3 692.34 and the average annual medical expenditure is RMB 3 716.29, the adjusted RR is 11.23; the average annual income of diabetes mellitus patients is RMB 4 497.63 and the average annual medical expenditure is RMB 4 297.95, the adjusted RR is 10.61. The average annum income of chronic bronchitis patients is RMB 3 578.90 and the average annual medical expenditure is RMB 2 919.01, the adjusted RR is 9.10. Only 32.41%, 38.71% and 24.29% patients of these three diseases have the ability to pay more than 20% medical treatment charge, and the abilities are very low. Conclusion： The economic burden and risk of patients with chronic burden are high among in rural area of Shanxi. The diabetics endure more than patients with hypertension and chronic bronchitis diseases; the abilities of patients with chronic disease are very low, and there is a trend of polarization between the rich and the poor.
