CCD联合VSD治疗远期复发性骨髓炎的分析

摘　　要：目的 探索慢性骨髓炎急性期治疗及控制远期复发率的有效方法，将传统的CCD（闭式灌洗引流closed continuous douche）与VSD（负压封闭引流术vacuum sealing drainge）相结合使用优点，观察临床治疗效果。方法回顾性将我院2004年1月至2011年9月收治的57例慢性骨髓炎患者，通过对其临床治疗资料分析，随机分为2组，对照组采用单纯闭式灌洗引流治疗，治疗组采用闭式灌洗引流联合负压封闭引流术治疗，分别于术后12-48月随访观察。对比两组患者术后中远期的临床疗效情况。结果术后57例患者在随访12～48月期间，按慢性骨髓炎的疗效评定标准评定，治疗组29例患者，痊愈22例，显效5例，有效1例，无效1例，总有效率为93．1％；对照组28例慢性骨髓炎患者中，痊愈13例，显效7例，有效5例，无效3例，总有效率为73．4％。两组治疗总有效率相比差异显著（P〈0．05）结论VSD联合CCD治疗慢性骨髓炎的复发率低，对于急性骨髓炎可提高治愈率。Objective To explore the effective method of treatment the acute stage of chronic osteomyelitis and control long-term recurrence rate, the advantages of the traditional CCD （closed continuous douche） combined with VSD （vacuum sealing drainge） to treat the acute stage of chronic osteomyelitis, to observe the clinical therapeutic effect. Method Reviewing the 57 cases of chronic osteomyelitis patients from 2004 January to 2011 September in our hospital, through the analysis of clinical treatment data, randomly divided into two groups, the control group were treated with simple CCD （closed continuous douche）, the treatment group were treated with CCD （closed continuous douche） combined with VSD （vacuum sealing drainge）, respectively 12 to 48 months follow-up observation after surgery, comparing the mid to Long-term clinical curative effects of the two groups after surgery. Result During the follow-up12 to 48 months observation after surgery, according to the evaluation criteria of chronic osteomyelitis, 29 cases of the treatment group, 22 cases were cured, 5 cases were markedly effective, 1 case was effective and 1 case was invalid, the total effective rate was 93.1%. 28 cases of the control group, 13 cases were cured, 7 cases were markedly effective, 5 case was effective and 3 cases was invalid, the total effective rate was 73.4%. The total effective rate of the two group was significant difference. Conclusion CCD （closed continuous douche） combined with VSD （vacuum sealing drainge） in the treatment the acute stage of chronic osteomyelitis with lower recurrence rate, it could also improve the cure rate of acute osteomyelifispatients.
