顺行髓内针治疗第五掌骨颈骨折的疗效观察

摘　　要：目的 探讨有明显移位的第五掌骨颈骨折的手术方法和临床疗效。方法2008年6月～2011年6月，应用顺行髓内针内固定术治疗了26例闭合性第五掌骨颈骨折的患者。结果所有患者术后随访6～10个月（平均8个月），骨折愈合后测得患侧头干角（15．3°±1．9°），健侧头干角（14．6°±1．7°），两组间比较差异无显著统计学意义（t=1．71，P=0．12）；术后患侧掌指关节（MCP）活动度范围（89．7±2．9），健侧掌指关节（MCP）活动度范围（90．7±1．5），两组间比较差异无显著统计学意义（t=1．83，P=0．10）。结论顺行髓内针内固定法是治疗第五掌骨颈骨折的方法，其效果可靠，值得推广。Objective To explore the clinical effect of operation and method in treating the neck of the fifth metacarpal fracture. Methods From June 2008 to June 2011, 26 cases of closed fractures of the fifth metacarpal neck were treated with antegrade intramedullary fixation. Results All patients were followed up for 6 to 10 months （average 8 months）, measuring the ipsilateral head angle （15.3 °±1.9 °） after healing, contralateral head angle （14.6 °± 1.7°）, there was no significant difference between two groups （t=-1.71, P=0.12）; postoperative metacarpophalangeal joint （MCP） range of motion （89.7±2.9）, contralateral metacarpophalangeal joint （MCP） ＇s range of motion （90.7± 1.5）, there was no significant difference between two groups （t=1.83, P=0.10）. Conclusion The effect of antegrade intramedullary fixation in treating the fifth metacarpal neck fracture is reliable, worthy of promotion.
