摘　　要：目的通过审核医院各科室不合理用药情况,为临床提供用药建议。方法根据药品说明书、相关法规及其他循证依据,判断病历用药是否合理,进行统计、分析,并将分析结果反馈于临床。结果神经系统状况的药物和中药注射剂不合理用药现象较为普遍,超适应证用药和超剂量用药较为明显。结论临床药师通过医嘱审核,提出不合理用药问题,逐步改善了临床用药合理性。Objective The auther analyzed the irrational use of drug with patients in each department of our hospital in order to make proper suggestions through exemplifications.Method The rational or irrational drug use was determined according to the instruction of drugs,relevant guidelines or regulations.Results The phenomenon of irrational drug utilization is relatively common in nervous system medicines,especially traditional Chinese medicine injections,No indications drug utilization and drug overdosage occured in some patients.Conclusion Clinical pharmacists can improve the rational drug application through reviewing orders,and putting forward suggestions.
